The Perioperative Period Risk Factors Of Pancfeaticoduodenectomy In Elderly Patients

Objective:This study was undertaken to investigate the relationship of postoperativecomplications after pancreaticoduodenectomy and perioperative correlativefactors. The results of the study can provide the basis for medical workers tomake preoperative assessment and reference index for elderly patients whoneed pancreatoduodenectomy.Methods:A total of50elderly patients(≥65years) were included in the finalanalysis.All the patients were given pancreatoduodenectomy.The risk factorsfor elderly patients were studied by univariate and multivariate Cox regressionanalysis.Results:In these50patients,14cases had postoperative complications (28%),2deaths (4%). Cox multivariate analysis display the independent risk factors ofpostoperative complications Include preoperative jaundice time, preoperativealbumin level, preoperative total bilirubin, BMI, Intraoperative blood loss andoperation time. Intraoperative blood loss, preoperative albumin levels andconcomitant diseases of major organ systems are independent risk factors for pancreatic fistula. intraoperative blood loss, preoperative total bilirubin,preoperative albumin level are independent risk factors for delayed gastricemptying.preoperative albumin level are independent risk factors forpostoperative bleeding.BMI, preoperative albumin levels, operation timeandimportant organ system complications are independent risk factors forinfection.preoperative jaundice time is an independent riskis an independentrisk factor for the MODS.Conclusion：advanced age is not a contraindications for pancreatoduodenectomy,Abundant preoperative assessment and perioperative period management canprevent postoperative complications effectively.
