Background:Thyroid cancer has been the most malignancy in endocrine surgery.It has seriously affected the healthy of human and the quality oflife.Surgery operation is the main means for thyroid cancer.Recurrentlaryngeal nerve injury is a common complication of,it is a main reasonfor medical malpractice. Intraoperative Neuromonitoring of Recurrentlaryngeal Nerve(IONM) can judge the recurrent laryngeal nerve injuryduring surgery by stimulating the upstream nerve (the vagus nerve or theupstream recurrent laryngeal nerve) and collecting the EMG signalproduced by the vocal cords. Therefore, it is favored by more and morethyroid surgeon. But whether it can reduce the operation time, blood lossand postoperative drainage volume, reduce the incidence of recurrentlaryngeal nerve injury, there is a lack of clinical evidence, pending furtherresearch.Experimental Purpose:This research evaluates the effect of IONM during thyroid cancerradical Operation by comparing the operation time, blood loss,postoperative drainage volume and postoperative vocal cord functionwith nIONM.Experimental methods:This research selected78cases who were diagnosed as UnilateralPapillary thyroid microcarcinoma from January1,2012to December31,2012. According to the Communication with patients and their families,they were divided into IONM group (36cases) and nIONMgroup (42cases).The IONM was applied to locate and protect therecurrent laryngeal nerve. The recurrent laryngeal nerve was located andprotected by being revealed.Then we compare the operation time,bloodloss,postoperative drainage volume,number of cleared lymph nodes, andpostoperative vocal cord function of the2groups.Results:1.IONM can effectively reduce the operation time.2.IONM can effectively reduce tissue damage.3.IONM does not significantly increase the number of the clearedlymph nodes.
