Objective:Observe the thoracolumbar burst fracture of the morphological characteristics of the Mid Column Injury, To explore correlation between neurologic deficit and the degree of Mid Column Injury in case of the thoracolumbar burst fracture.Methods:88case of thoracolumbar spine burst fractures were retrospectively analysized. It was recorded the clinical data for each patient, including nerve function, the midsagittal diameter on CT scan to injuried spine, and other related indicators. According to Frankel score classification, it was excluded the dysfunction due to the spinal cord concussion in short-term. ImageJ image processing software was used to measure the sagittal diameter for the rate of spinal canal encroachment. Statistical analysis including descriptive analysis and pearson correlation was respectively performed at different Mid Column Injury form thoracolumbar burst fracture.Results:1, According to Mid Column Injury in the form of CT image features,88patients were divided into four categories, After the top of the shift type,Central shift type, Free shift and the overall shift type. On the After the top of the shift type in37cases (accounting for42.04%of the total number of cases), Central shift type in29cases (accounting for32.95%of the total number of cases), Free shift type in12cases (accounting for13.63%of the total number of cases), the overall shift type in10cases (accounting for11.36%of the total number of cases).2, Were56patients with nerve injury occurred, on the After the top of the shift type in29cases (accounting for51.78%of the total number of cases of nerve injury), Central shift type in12cases (21.42%of the total number of cases of nerve injury), Free shift type in11cases (19.64%oof the total number of cases of nerve injury), the overall shift type in4patients (7.14%of the total number of cases of nerve injury). Calculated for4type of the spinal canal encroachment rate Frankel spinal cord function score between the correlation coefficient (r value) were:0.846,0.518,0.858and0.618, a significant test (P value), respectively:P<0.001,0.05<P<0.1, P<0.001and0.1<P <0.2. After the top of the shift type and free shift type spinal fracture block occupation rate and extent of spinal cord injury after a high correlation of the statistical results suggest that, rather than Central shift type and overall shift type between the two low. Correlation coefficient, significance test results showed that in the After the top of the shift type and free shift type between the linear correlation (both P<0001), in the Central shift type and overall shift type between the linear correlation (P=0.05<P<0.1,0.1<P<0.2).Conclusion:Thoracolumbar burst fractures observed on the CT image data, divided into4types according to the column injury morphological characteristics, After the top of the shift type,Central shift type, Free shift and the overall shift type.Which the upper part of the fracture in the column to the spinal displacement type were more common, and accompanied by the largest proportion of cases of nerve injury.Spinal fractures occupation rate and degree of spinal cord injury correlation, the correlation between After the top of the shift type and Free shift type.
