Objective:Discuss the clinical and laboratory indicators and find out independent riskfactors which can influence the prognosis of patients with liver failure, provideevidences for clinical diagnosis and treatment.Method:Retrospectively analyze304patients with liver failure who stayed in No.1Hospital of Jilin University from January1,2000to January1,2013.All the caseshave complete medical records and follow-up results. Record the clinical andlaboratory indicators of all the patients. According to the first3months afteradmission, all the patients are divided into survival group and death group, comparetheir prognosis. Taking the indicators which are statistically significant into theLogistic regression analysis, screen out independent risk factors which influence theprognosis of patients with HBV related liver failure.Results:1. The survival rate of liver failure caused by HBV is39.53%, and the survivalrate of liver failure caused by all other factors except HBV is52.27%, the differencebetween the two survival rates is statistically significant(P<0.05).2. The survival rate of acute liver failure is39.62%, the survival rate of sub-acuteliver failure is44.12%, the survival rate of acute (sub-acute) on chronic liver failure is52.10%, the survival rate of chronic liver failure is46%. The difference between thesesurvival rates of each group are not statistically significant(P>0.05).3. Independent risk factors for the prognosis of patients with HBV relatedhepatic failure are: total bilirubin (TBIL), prothrombin activity (PTA), serum sodium (Na+), total cholesterol (TC), CTP score, age50or more, the presence of livercirrhosis, bilirubin enzyme separation, complications (especially hepaticencephalopathy, hepatorenal syndrome and infection).Conclusion:Independent risk factors for the prognosis of patients with HBV related hepaticfailure are: total bilirubin (TBIL), prothrombin activity (PTA), serum sodium (Na+),total cholesterol (TC), CTP score, age50or more, the presence of liver cirrhosis,bilirubin enzyme separation, complications (especially hepatic encephalopathy,hepatorenal syndrome and infection).
