The Abnormal Expression Of BCL-2, MYC In Aggressive B Cell Lymphoma

ObjectivesTo study the relativity between the independent or combined abnormal expression ofBCL-2，MYC gene and clinical-associated important factors, clinical effects and survivaltime.MethodsFISH was used to detect the abnormal expression of BCL-2, MYC gene in55cases ofaggressive B cell lymphoma tissue paraffin-embedded specimens, more15cases oflymphocytic inflammatory hyperplasia were used to establish anomaly threshold.ResultsIn the55cases of aggressive B cell lymphoma tissue,18were BCL-2amplification,amplification rate was32.73%(18/55),16cases were MYC aneuploid, incidence rate was41.82%(23/55).1was BCL-2/IGH, rearrangement rate was1.82%(1/55),7cases wereMYC/IGH, rearrangement expression rate was12.73%(7/55).8cases were BCL-2combinedMYC abnormal expression, combined abnormal expression rate was14.55%(8/55)%，of the8cases，1was BCL-2/IGH combined MYC/IGH (Double hit lymphoma、DHL), incidencerate was1.82%(1/55). BCL-2abnormal expression was correspondence with high β2-MG(>1.8mg/L) and high IPI (3~5score)(P<0.05). The rate of high β2-MG and high IPI in MYCabnormal group were higher than normal group, but no significant differences in two groups.In the Subgroup of IPI, BCL-2abnormal expression was correspondence with high LDH(>226U/L), MYC abnormal expression was correspondence with Ⅲ~Ⅳ clinical stages(P<0.05). Compare with normal group, the clinical effects in BCL-2abnormal group waspoor (P<0.05). Median survival in the patients with BCL-2abnormal expression was11.4months，with MYC abnormal expression was6.4months. The Hepatitis B virus infection ratein BCL-2abnormal group and normal group were77.78%(14/18) and45.95%(16/23)respectively，the difference between both groups was statistically significant (P<0.05), therewere no significant differences in MYC abnormal group and normal group． Conclusions1. Of the55aggressive B cell lymphoma tissues, The abnormal expression of BCL-2gene is32.73%,MYC gene is41.82%，combined abnormal expression of BCL-2and MYCgene is14.55%, the incidence of DHL is1.82%.2. The abnormal expression of BCL-2ischiefly related to high LDH and high β2-MG.3. The abnormal expression of BCL-2is relatedto high β2-MG, tends to yield poor clinical effects.3. The abnormal expression of MYC ischiefly related to later clinical stages, and mainly embodied with lower treatment of totalreaction rate (ORR).4. Compared to patients who have on abnormal gene expression, theabnormal expression of BCL-2and MYC are chiefly related to short lifetime, patientscombined abnormal expression of BCL-2and MYC gene were shorter than patientsindependent abnormal expression of BCL-2、MYC gene.6. The group of BCL-2abnormalexpression is consistent with high detection rate of hepatitis B virus, deduce hepatitis B virusnot only through impress autoimmune to involved in the development of lymphoma, butalso might be take part in the change of molecular biology.
