ObjectiveTo clarify the value of the narrow band imaging(NBI) system intissue characterization and differential diagnosis.MethodsA series of75patients with116colorectal lesions were studide. According to Yoshiki Wada’s type method, The vascular patterns of colorectal lesions include six species: honey-comb,faint,network,dense,irregular,sparse. During NBI, the microvascular architecture observed on the surface of the detected lesions. Results ofNBI were compared with the histological results.ResultsMost hyperplastic polyps showed a faint pattern. The vascularpatterns of adenomas were mainly the network pattern or the densepattern. The major vascular patterns of cancers were the irregularpattern and the sparse pattern. When we assumed that the faintpattern was diagnostic for hyperplastic polyps, we could differentiatebetween neoplastic and non-neoplastic lesions with a sensitivity of94.6%and a specificity of78.6%. The accuracy, positive predictivevalue and negative predictive value of capillary patterns (CP) underNBI were88.8%,88.6%and89.2%(P<0.01). Likewise, irregular andsparse patterns were assumed to be indices of cancer, the sensitivitywas100.0%, the specificity was87.5%, and the accuracy rate was91.4%,(P<0.01). Lesions<10mm were classified into2groups: polyps with invisible or faintly visible meshed capillary (MC) vessels asnonneoplastic and polyps with clearly visible MC vessels as neoplastic.the sensitivity, specificity, positive predictive value, negativepredictive value and accuracy were89.7%,80.5%,81.4%,89.2%and85.0%(P<0.01).ConclusionThe NBI system was valuable for distinguishing betweenneoplastic and non-neoplastic lesions, as well as between cancersand adenomas. The NBI system was valuable for differential diagnosisof lesions <10mm.
