Aims:This paper is to know the occurrence and development of N-cadherin in hilar cholangiocarcinoma, and to definite the expression and significance of hilar cholangiocarcinoma、carcinoma sides tissues and normal bile duck tissues,and to measure if it can be used as the prediction index of invasion、metastesis and prognosis in hilar cholangiocarcinoma.Methods:Collect20Paraffin-embedded samples of cancerous tissue and adjacent tissues from the patients with hilar cholangiocarcinoma who did surgical resection in our hospital during2006to2011, and20Paraffin-embedded samples of normal bile duct within the liver after surgical resection of hepatic cavernous hemangioma in our hospital.The adjacent tissues are cancer weeks, pathological confirmation of non-cancerous tissue infiltration of the gallbladder, liver, omentum, round ligament of liver or lymph nodes.20cases of carcinoma sides tissues(not be invasived by tumor) and20cases of normal bile duck tissues were examined with S-P immunohistochemieal (IHC) technique. In those cases, there are15patients of adenocarcinoma,2patients of mucinous adenocarcinoma and3patients of tubular adenocarcinoma;8patients have lymph node metastasis;16patients have perineural invasion.According to TNM stage,4patients belong to Ⅰ stage,2patients belong to Ⅱ stage,8patients belong to Ⅲ stage,6patients belng to Ⅳ stage. All patients had not received chemotherapy、radiotherapy and bio-immunotherapy therapy, and make an further image quantitative analysis of the expression intensity of N-cadherin using Image-pro plus (IPP6.0) quantitative image analysis software, and describe its positive expression intensity by using mean optical density(MOD). Furthermore, to compare the difference among these three group about the positive intensity. It is more objective than manual count. And to make a further prove of the reliability of the result of Immunohistochemistry. The differences of N-cadherin in10cases of hilar cholangiocarcinoma and6cases of carcinoma sides tissues were examined with reverse transcription polymerase chain reaction(RT-PCR).All Selected cases contained in the chemical are immunohistochemistry of frozen tissue, and adjacent tissues are tumor without infiltration of the gallbladder,and then make statistical analysis of the ratio of the bands and the selected bands of internal reference gray scale.Results:After using the manual count to analyse immunohistochemistry pathological chemical biopsy, it shows that the positive expression of N-cadherin in hilar cholangiocarcinoma、 carcinoma sides tissues and normal bile duck were to be respectively. The positive expression of N-cadherin in hilar cholangiocarcinoma、carcinoma sides tissues and normal bile duck were80%、20%and0%.Normal bile duck and carcinoma: X2=26.667, P<0.01, there were significant differences;carcinoma side tissues and carcinoma:X2=14.400, P<0.01, there were significant differences. The result of experiment shows:significant correlation was observed between high N-cadherin expression and degree of differentiation,TNM stage and nervous invasion(P< 0.05),and no difference could be found between N-cadherin expression in age, sex of patients、histological type and lymph node metastasis(P>0.05).The result of image quantitative analysis shows:There are differences between the mean optical density of carcinoma and normal bile duck(P<0.01). There are differences between the mean optical density of carcinoma and carcinoma sides tissues.(P<0.01) There were no differences between carcinoma sides tissues and normal bile duck(P=0.349).The result of RT-PCR shows:There are differences between the content of mRNA about N-cadherin in carcinoma and carcinoma sides tissues (P <0.01). Conclusions:The increased of N-cadherin expression can be used as indicators of the assessment of hilar cholangiocarcinoma invasion, metastasis and prognosis.
