BackgroundAllergic rhinitis (AR) is a chronic inflammation disease, by IgE-mediated and a variety of immunocompetent cells and cytokines involved, which occurs in nasal mucosa. The major clinical manifestations are nasal congestion, runny nose, nasal itching, sneezing, as well as olfactory dysfunction, belonging to the scope of Chinese medicine,"Allergic Rhinitis". In present, AR is more and more prevalent in whole world. In aboard, the study report that14%person was diagnosed has allergic rhinitis, and some countries, self-reported prevalence of up to45%. In china, the self-reported prevalence in eleven cities was8%to21.4%. Allergic rhinitis can lead to tympanitis, the trachea and bronchitis, asthma and have a serious impact on the patient’s sleep, quality of life. Therefore, prevention and treatment of allergic rhinitis is very important.ObjectTo investigate the effects of acupuncture on the serum IL-4and IFN-y and ultrastructure of nasal mucosa in SD rats with allergic rhinitis, Provide laboratory evidence for acupuncture treatment of allergic diseases.Method All48SD rats were randomly selected12rats used as the control group, and other36were used for making AR model. The36SD rats were use the VOA made the AR model and all the36rats were demonstrated nasal congestion, runny nose, scratching nose symptoms, indicating that the model was successful, and then randomly divided into three groups:acupuncture group, modeling group, Fluticasone propionate nasal spray group,12rats in each. Rats in the acupuncture group were treated with acupuncture on da zhui, fei shu and shenshu once a day for30min. Rats in the Fluticasone propionate nasal spray group were treated with Fluticasone propionate nasal spray, once a day,50ug each nostril. Rats in the modeling group and control group were not interfered. After10days of consecutive treatment, nasal mucosa were taken for HE staining and transmission electron microscopy(TEM), Serum was used to detect IL-4and IFN-γ.ResultThe IL-4level in control group, modeling group, Fluticasone propionate nasal spray group and acupuncture group were12.27±1.49Pg/ml,38.84±2.45Pg/ml,26.18±1.57Pg/ml,32.94±1.81Pg/ml, Significant differences among the four groups (P<0.001). Modeling group’s level was higher than control group’s, the difference was statistically significant (P<0.05); Fluticasone propionate nasal spray group’s and acupuncture group’s level were higher than control group’s and lower than modeling group’s, the difference were have statistically significant (P<0.05); acupuncture group’s level was higher than Fluticasone propionate nasal spray group’s, the difference were have statistically significant (P<0.05); it shows that the serum IL-4levels in allergic rhinitis rats was elevate, the acupuncture and glucocorticoid can inhibited it, but glucocorticoid effect is more obvious.The IFN-y level in control group, modeling group, Fluticasone propionate nasal spray group and acupuncture group were40.03±1.85Pg/ml,21.86±1.50 Pg/ml,35.17±1.30Pg/ml,30.36±1.47Pg/ml,Significant differences among the four groups (P<0.001). Modeling group’s level was higher than control group’s, the difference was statistically significant (P<0.05); Fluticasone propionate nasal spray group’s and acupuncture group’s level were higher than modeling group’s and lower than control group’s, the difference were have statistically significant (P<0.05); acupuncture group’s level was lower than Fluticasone propionate nasal spray group’s, the difference were have statistically significant (P<0.05); it shows that the serum IL-4levels in allergic rhinitis rats was decrease, the acupuncture and glucocorticoid can elevate it, but glucocorticoid effect is more obvious.The EOS in control group, modeling group, Fluticasone propionate nasal spray group and acupuncture group were0.75±0.25,11.17±0.68,1.67±0.31,3.00±0.35, Significant differences among the four groups (P<0.001). Fluticasone propionate nasal spray group’s level were lower than modeling group’s the difference were have statistically significant (P<0.05), but has no statistically significant with control group’s; Acupuncture group’s level was lower than modeling group’s and higher than Fluticasone propionate nasal spray group’s, the difference have statistically significant (P<0.05); it shows that the EOS in allergic rhinitis rats was elevate, the acupuncture and glucocorticoid can inhibited it, but glucocorticoid effect is more obvious.Samples from control group present integrated ultrastructure, intact well-arranged cilia and microvilli, abundant and normal. Samples from modeling group showed that the epithelium were disconnected, cilia and microvilli were adhered, lodged and even desquamated. The structures of organelles were changed with vacuolization. Cell junctions were swollen. Samples from Fluticasone propionate nasal spray group appeared less injured compared with modeling group. It can’t see Cilia and microvilli adhered and lodged. The cell junctions were almost normal. Samples from acupuncture group appeared less injured compared with modeling group but more injured compared with Fluticasone propionate nasal spray group. Cilia and microvilli were adhered and lodged. Cell junctions were slightly swollen.ConclusionIn allergic rhinitis rats, the nasal mucosa with massive infiltration of eosinophils, epithelial pathological serious damage, the serum IL-4level were elevated than normal rats and the IFN-y level were lower than normal rats. It suggested that Th1/Th2imbalance is one of the pathogenesis of allergic rhinitis, EOS infiltration, cilia system damage is allergic rhinitis the local pathological damage.Both acupuncture and glucocorticoid can decreased the serum IL-4level and increased the IFN-y level, as well as inhabited the EOS invade in Nasal mucosa, suggesting that acupuncture may be through reducing EOS infiltration of nasal mucosa, thereby improving the pathological damage and the mechanism in therapy allergic rhinitis may be corrected via Th1/Th2imbalance to achieve.Effect of acupuncture on the treatment of allergic rhinitis was lower than glucocorticoi, the exact cause is still need to be further studied. it probably the model rats after allergy is more intense, so acupuncture through immune regulation takes a long time, or it may be both mediated inflammatory mechanisms are not the same wav.
