Objective In this paper, through the collection of clinical intertrochanteric fracture cases to analyze the treatment method and the curative effect of anti rotation, comparison of proximal femoral nail antirotation (PFNA), locking proximal femurs plate (LPFP) and conservative treatment of clinical effects of three treatment methods, and provide basis for further study, and for the femur intertrochanteric fracture treatment experience.Methods From June2010to March2012in Chengdu University of TCM Hospital orthopedic. and Tianquan Hospital of Traditional Chinese Medicine treated a total of over50years old patients with intertrochanteric fracture in78cases, of which26cases were treated with PFNA internal fixation,26cases with locking proximal femurs plate (LPFP) internal fixation,26cases with conservative treatment of bone traction. To observation the treatment of patients in operation time, intraoperative bleeding, the length of the incision, postoperative complications, lower time, hospitalization time and other indicators, and fracture healing time, suffering from hip function, according to the Harris hip score. The results were statistically analyzed by spss17.0, evaluation of three different treatment methods for clinical effect.Results In this group78cases in all, all patients were followed up to fracture healing for8-14months, with an average of9.35±2.05months, non-fatal cases. According to Harris hip score of post-traumatic function criteria, the satisfactory rate was88.5%in PFNA group, higher than the LPFP Group’s76.9%, and The satisfactory rate of the conservative group was42.3%. Three groups of postoperative functional restoration of hip joint following aspects of the differences were statistically significant (P<0.05), PFNA group of postoperative functional restoration of hip joint following the final good, LPFP group. PFNA group and LPFP group in the operation time, intraoperative bleeding, operation incision length and had a significant difference (P<0.05), PFNA group than in LPFP group, operation time is short, small incision, less bleeding during operation. The three groups in the postoperative complications in aspects of the differences were statistically significant (P<0.05), PFNA group the postoperative complication rate is lowest, LPFP group. The three group during the hospitalization time, next time, fracture healing time compared the difference was statistically significant (P<0.05), but the PFNA group and LPFP group during the hospitalization time difference was not significant (P>0.05), operation group hospitalization time was considerable, but significantly less conservative group; group PFNA next time the shortest, LPFP group; at the time of fracture union, a conservative group shortest, PFNA group.Conclusion This study believes that intertrochanteric fractures should be treated by operation early, in order to reduce complications and lower mortality rate, which can reduce the hospitalization time, distribution of medical resources reasonable; The PFNA treatment can reduce the operation time, less bleeding, early ambulation and functional exercise earlier, promoting fracture healing, promote functional recovery of the hip joint. The PFNA is ideal for internal fixation materials, in line with the BO concept, worthy of clinical application.
