Abstract/Summary:
Objective:By enzyme linked immunosorbent assay method for the detection of endometriosis in theserum of patients with human epididymis secretory protein4(human epididymis protein4,HE4protein) expression; by chemiluminescence detection of endometriosis in sera of patientswith CA125expression; by immunohistochemical SP method in endometriosis tissue ofpatients with HE4protein expression of HE4protein; in endometriosis and its role inpathogenesis, and in endometriosis diagnosis and differential diagnosis value of.Methods:Using biotin double antibody sandwich ELISA assay for detection of86cases ofendometriosis patients,56patients with epithelial ovarian cancer patients,26cases of benignovarian epithelial tumor patients serum and20cases of healthy women in the serumHE4protein content; using chemiluminescence detection of all cases serum CA125content;the immune group of streptomycin avidin-peroxidase (S-P) method for the detection of thecorresponding86endometriosis patients,56patients with epithelial ovarian cancer and26casesof benign ovarian epithelial tumor tissue of patients with HE4protein expression.Using the SPSS17.0statistical software for statistical analysis, measurement data to x±s,using t test and variance analysis, hierarchical information using frequency counts, groupswere compared using Ridit correlation analysis, using Spearman correlation analysis, thecorrelation coefficient will express with rs, P <0.05, with statistical significance.Results:1.the expression of HE4protein in endometriosis, ovarian epithelial carcinoma andbenign ovarian epithelial tumor tissueEndometriosis is mostly visible more complete gland, epithelial compression, a shrinking sample. HE4protein in endometriosis and ovarian epithelial tumor tissue expression ofmultiple negative, and in the epithelial ovarian carcinoma tissue in the positive rate was78.57%(22/28), mainly in the cytoplasm and nucleus, expression intensity for++~+++;there was a significant difference (P=0.005).2.the determination of HE4protein content in serum of Endometriosis, ovarian cancer,benign ovarian epithelial tumor patients and normal controlsPatients with endometriosis preoperative serum HE4protein mean58.562±18.147pmol/L(50.415~66.709pmol/L), epithelial ovarian cancer in patients with preoperative serum HE4protein mean187.184±89.915pmol/L (167.269~507.099pmol/L), benign ovarian epithelialtumor patients with preoperative serum HE4protein value52.629±22.277pmol/L (40.352~64.906pmol/L), normal human serum HE4protein mean50.059±27.214pmol/L (42.845~57.273pmol/L). Patients with endometriosis preoperative serum HE4protein with benignovarian epithelial tumor group and normal control group compared with slightly elevated, thedifference was not statistically significant (P>0.05). Epithelial ovarian cancer in patientswith preoperative serum HE4protein with endometriosis and control group were significantlyhigher (P <0.01),150pmol/L as the boundary values, serum HE4positive rate is57.69%.3.Serum HE4protein content in patients with epithelial ovarian carcinoma age,pathological grade, clinical stage, lymph node metastasis and ascites unrelated (P>0.05), andovarian serous adenocarcinoma of the serum level of HE4was significantly higher than that ofserous (P=0.044).4.Endometriosis, ovarian cancer, benign ovarian epithelial tumor patients and normalcontrols in the serum CA125content determinationPatients with endometriosis preoperative serum CA125mean81.523±32.091U/ml(12.439~113.621U/ml), epithelial ovarian cancer in patients with preoperative serum CA125mean298.13±117.346U/ml(56.804~965.496U/ml), benign ovarian epithelial tumor patientswith preoperative serum CA125mean of28.819±15.482U/ml(6.408~62.372U/ml), normalserum CA125mean10.117±6.846U/ml(6.264~13.956U/ml). Endometriosis group serumCA125levels were significantly higher than those of benign ovarian epithelial tumor groupand normal control group, the difference was statistically significant (P <0.01). Epithelialovarian cancer group serum CA125levels were significantly higher in the endometriosisgroup and other groups, the difference was statistically significant (P <0.01). Serum CA125 to50U/ml as the boundary values, endometriosis, the positive rate was60.12%, with100U/ml as the boundary values, epithelial ovarian cancer serum positive rate of CA12565.85%,and HE4in serum and CA125combined detection of epithelial ovarian cancer with positiverate of76.70%.5.The content of CA125in serum and epithelial ovarian cancer patients age, histologicalgrade, lymph node metastasis and ascites unrelated (P>0.05), and ovarian serousadenocarcinoma of the serum level of CA125was significantly higher than that of serous (P=0.016), ovarian serous adenocarcinoma III~IV was significantly higher than that of I~II (P=0.011).6.Endometriosis and ovarian epithelial tumor patients serum HE4protein content andtissue expression of protein HE4in no correlation (rs=0.214, P <0.05; rs=0.328, P>0.05);while the epithelial ovarian cancer patients with serum HE4protein content and organization ofHE4protein is highly linear correlation.Conclusion:1.Endometriosis patients serum and tissue HE4protein in ovarian epithelial tumorexpression similarity. Serum HE4protein in endometriosis diagnosis have reference value.2.Epithelial ovarian cancer patients in the serum and tissue protein expression wassignificantly higher than that of HE4, endometriosis and ovarian epithelial tumor, with150pmol/L as the boundary value of epithelial ovarian cancer patients, the positive rate ofHE457.69%in serum. And serum HE4protein content and organization of HE4protein ishighly linear correlation. Level of serum HE4protein in endometriosis and ovarian epithelialcancer in the differential diagnosis of a certain significance.3.Endometriosis patients with serum CA125levels than benign ovarian epithelial tumor;epithelial ovarian cancer patients with serum CA125levels were significantly higher inendometriosis and ovarian epithelial tumor. Serum CA125to50U/ml as the boundary values,endometriosis, the positive rate was60.12%, with100U/ml as the boundary values,epithelial ovarian cancer serum positive rate of CA12565.85%, and HE4in serum and CA125combined detection of epithelial ovarian cancer with positive rate of76.70%. Description ofjoint detection in differential diagnosis of benign and malignant pelvic mass, there is referencevalue.
