| Purpose and significance of the researchCerebral palsy (abbreviation as CP as following) is one of the most common diseases that cause child’s disability and it has a bad impact on the quality of our population. Until now, there is still no method to cure this disease. All kinds of rehabilitation and treatment could only reduce child’s degree of disability. According to the estimate of National Cerebral Palsy Meeting, domestic morbidity of CP reaches around1.8‰-4.0‰. If we suppose there are20million newborn babies every year, then we will have about3.6-8million CP children. It’s a really big number which will lay huge economic burden,psychological pressure and pains on both family and society. The situation is really serious.Domestic rehabilitation of CP depends too much on professional rehabilitation in medical institutions, which caused patients and their parents travel around to find best hospital and spend a lot of time and money on rehabilitation. However, CP is a sequela of damage and degeneration of brain tissue. It means that rehabilitation for CP is not only sheerly rehabilitation to shape body or recover body function. Current mode has a lot of advantage that it could only cover a few population and it neglect communal rehabilitation, family rehabilitation which will retard psychological and other progress of patient.To solve these problem and facing significance of CP rehabilitation, Shanghai Disabled Persons’ Federation carried out new allowance policy in2007and2009. Basing on guaranteeing and promoting institutional rehabilitation for CP, the policy extend family rehabilitation and put forward comprehensive rehabilitation and intervention mode.My essay is mainly about institution-community-family rehabilitation and put forward comprehensive rehabilitation of CP children while advocating continuous rehabilitation. It points out that parents are not only carer of disabled persons and community are not only administer of disability affairs. They could both be a part of rehabilitation and be active in it. The thoughts and methods of this research have some innovation in it and it could be regarded as an exploration of rehabilitation and a reference to rehabilitation for other disabilities. Materials and MethodsBy reading and analyzing academic documents as well as policy documents, I have a general idea of domestic and foreign situation of CP rehabilitation. Then I made three booklets (evaluation booklet of family rehabilitation for CP children, family rehabilitation card for CP children and guiding handbook of family rehabilitation for CP children).During the process of analysis, we established database by Epidata3.0and analysed it by Spssl7.0. If P is less than0.05, then it’s meaningful in statistics. Furthermore, we controlled quality of research by training and guiding.Main results1、Research on the present status on CP children(1) In developed countries as European countries, America and Australia, there are strong community-based rehabilitation for CP children, which include medical, educational, vocational and social rehabilitation. They integrated hospital rehabilitation with rehabilitation out of hospital and combine these with sound legal system, administration and service net and a series of evaluation system.(2) There are12-14thousand newborn children in Shanghai every year. According to morbidity of CP as1.8‰-4.0‰, there are216to560CP children every year. We log in the database of disabled children of network platform of Shanghai Disabled Persons’Federation and found that there were7311disabled children to have rehabilitation until Nov.31,2011, among which there are1974children(1124boys and850girls to have CP rehabilitation)2、Sociological statistics of parents of CP children(1) Life quality analysis:When I analysed three factors as training period, GMFCS grade of CP children and education background of their parents, extension of training period could enhance life quality of patients and their parents, especially in four dimensions as body function, vitality, emotional character limit and psychological health. The other two factors have no direct impact on life quality of parents.(2) Score of social support rating scale:The score of parents who have one period of training are better than those have several periods, but not significantly different. When the first score is compared with score of the parents on the first period, it has significant difference on total scores and other statistics.3. Evaluation of rehabilitation effect by parents (1) Evaluation of training effect:According to statistics, there are124persons that filled in points of training effects with average as6.48and the median as7. We concluded by a medium training effect. It shows the evaluation tend to significantly improve following the progress of training.(2) degree of satisfaction:According to statistics, there are124persons that fill in degree of satisfaction in summary with average of6.85and the median as7. It shows the degree of satisfaction tend to significantly improve following the progress of training.(3)Cooperation degree of parents:According to statistics, there are266persons that graded cooperation degree of parents. Among these degrees, the lowest is4and9persons gave this point. The highest is10and17gave this point. The average is7.5. It shows that parents tend to cooperate following the progress of training.(4) Effect evaluation of medical intervention:All the children’s GMFM score tend to significantly improve afrer3months or more training.According to their different GMFCS grade and age,the GMFM score also tend to significantly improve.Conclusion1、Comprehensive rehabilitation and intervention for CP children are quite necessaryComprehensive rehabilitation could have active effects on training effect, degree of satisfaction, cooperation degree. Medical rehabilitation shows its professional effects in the whole system while family rehabilitation is important supplement and community-based rehabilitation is important guarantee.2、Effects and value of establishing comprehensive rehabilitation mode which is guaranteed by policiesConsidering policy cost, comprehensive rehabilitation and intervention policy has a low cost. While considering implement of policy, government plays a key role as bond and administer that could take advantage of social resource to give technical and financial supports for rehabilitation. It is in accordance with current situation of development of rehabilitation in this city. It’s a necessary way to realize comprehensive rehabilitation that managed by the government and participated by society and family. |
