Objective: To investigate the factors which impact the surgical outcome ofextramedullary spinal cord lesions (ESCLs), we analyzed the clinical materials ofsuch patients retrospectively. Further comparisons of the outcome and resection rateof ESCLs were also performed.Materials and Methods: A total of119patients of ESCLs (88males and111females;mean age33.6±3.1years; duration of symptoms,25.6±2.4month) underwentoperation between Jan2005and Dec2011were investigated. Preoperative/Postoperative and follow-up status of patients were classified by the McCormickgrade. The extent of tumor resection, tumor parameters (maximum anteroposteriordiameter/longitudinal length/stage distribution/directions), postoperativecomplications, pathological outcomes and recurrence were investigated. The factorswhich may impact the outcome (sex, age, preoperative/postoperative functional status,extent of tumor resection, tumor parameters) and pathological classification wereanalyzed by the logistic regression. The differences of total resection rate andoutcome between pathological groups were compared by Chi-square test.Results: Preoperative clinical functional is the most important factors which impactthe surgical outcomes of ESCLs (OR=12.10,95%CI=3.25–46.80, p=0.002). Thereis stastistical difference in the total resection rate and outcome between pathologicalgroups(χ2=19.67,P<0.01;χ2=23.64,P<0.01).The total postoperative resection rateis90.4%in the neurilemoma group, and the excellent rate is85.0%in thefibroneuroma group.Conclution: Most of the extramedullary spinal cord lesions are benign. Surgicalintervention has a high success rate for tumour control and functional improvement inmost cases.
