Blood is an important liquid tissue. If severe trauma, surgery, or esophageal varicesrupture occurs, a lot of blood will loss in a short period of time, which may lead to severehemorrhagic shock. Fluid resuscitation, as an important measure, is needed to restorecirculating blood volume and maintain perfusion pressure. Unfortunately, currentlyresuscitation fluids can cause organ system injury. Pyruvate is the simplest α-keto acid,and its sodium salt has many advantages during hemorrhagic shock. However, pyruvate isquite reactive and reacts via aldol condensation to form dimer. And the dimer of pyruvatecan inhibit the tricarboxylic acid cycle in mitochondria. There is yet not the report aboutwhether the mixture of sodium pyruvate and its dimer still has inhibitory effect on cells oranimals. As to this, we study it on cells through some works as below shown:(1) An ion-pair reverse phase HPLC is established for quantitative analysis sodiumpyruvate in aqueous solutions. Meanwhile, we analysis the sodium pyruvate condensationreaction rate after the pH is adjusted to12with NaOH.(2) We purify the dimer by acetic acid, and its relative content in percentage is above98%by area normalization method analysis.(3) Human umbilical vein endothelial cell is selected as the experimental cells and wefind that the mixture of sodium pyruvate and its dimer can result in the decrease of OD570value (MTT assay), which is positively correlated with the concentration of the mixture,dimer proportion and duration of incubate time. Furthermore, even if using the mixturecontaining highest proportion dimer still can not induce apoptosis, and also can maintainthe mitochondrial membrane potential at a high level. Meanwhile, it can make the contentof intracellular reactive oxygen species decline. All the results abtained above indicate thatthe mixture can not cause significant damage on cells. Therefore, if the dimer is kept in alower content and the quality standards are also established, sodium pyruvate is verylikely to be a new resuscitation fluid.
