The Influence Of Yishen Jiangzhuo Granule On The Bone Metabolism And Il-17in Patients With Chronic Kidney Disease Stage3-4

Objective:To detect the change of Yishenjiangzhuo granules on tartrate resistant acid phosphatase(TRACP), C-reactiveprotein(hs-CRP), and regulatory T cells(Treg) in patients with chronic kidney disease(CKD) stage3-4before and after treatment. To explore the influence of Yishenjiangzhuo granule on bone metabolic indicators and micro-inflammation in patients of CKD stage3-4.Methods:According to the standard of NKF-K/DOQI proposed,fifty-three patients with chronic renal disease were randomly divided into YSJZ group and control group. Serum level of TRACP、IL-17was determined by ELISE assay. Serum level of hs-CRP was determined by scattering turbidimetric analysis. The level of peripheral blood Treg (CD4+CD25+CD1271o) was detected by three-color fluorescent labeling flow cytometry. Hemoglobin (HGB) content by Beckman-Coulter hemo-analysis; urinary contents of creatinine (UCr) by inverse HPLC; blood contents of calcium (Ca),phosphate (P),blood urea nitrogen (BUN),creatinine (Scr) by automatic biochemical analyser. Then the product of calcium-phosphate (CaxP)was calculated based on blood contents of Ca and P,and the clearance rate of endogenous creatinine (CCr) was calculated based on blood BUN and SCr.Results:①Compared with normal group, levels of TRACP in both groups before treatment were higher,there was significant change; but after treatment were still higher in both groups (P<0.05).②After treatment, Ca increased in the two groups (P<0.05), but the treatment group had no significant difference between the control group (P>0.05); The levels of P and calcium-phosphorus product had no significant change After treatment,(P>0.05).③Compared with normal group, levels of TRACP in both groups before treatment were higher,there was significant change; After treatment were still higher in both groups,the levels of IL-17showed no significant change after treatment in the YSJZ group (P>0.05), showed significant changes after treatment in control group (P<0.05).④After YSJZ treatment the levels of hs-CRP were lower in both groups,but showed no significant change.⑤After treatment,the level of Treg had no significant change in the two groups(P>0.05), but Compared with normal group showed significant changes.⑥After treatment Scr decreased in both groups (P<0.01),while the YSJZ group decreased more significantly (P<0.05).While CCr increased in both groups (P<0.01),while the YSJZ group increased more significantly (P<0.05).⑦Correlation analysis:Levels of IL-17、TRACP、 hs-CRP、Treg were unrelated.Conclusions:1. Levels of TRACP were increased, and bone metabolic indicators disordered in patients with chronic kidney disease(CKD) stage3-4.2. Levels of IL-17、 hs-CRP were increased in CKD stage3-4patients, who were in the state of mieroinflammation.3. Yishen Jiangzhuo Granule could improve kidney function and delay the process of micro-inflammation but could not improve bone metabolic.4. Inflammatory factors in patients with CKD Stage3-4involved in inducing osteoclasts activation.
