Objective:To observe the clinical efficacy of the injection of salvia tetramethylpyrazine on patients with craniocerebral injury, and to explore its possible mechanism of action. To study the application of the injection of salvia tetramethylpyrazine on changes of serum brain natriurific peptide(BNP) and neuron specific enolasc(NSE) levels in patients with craniocerebral injury.To explore the relationship between BNP and NSE levels, the severity of traumatic brain injury and prognosis.Methods:60patients who are diagnosed craniocerebral injury(GCS score<12points)were divided into two groups at random,the western medicine group and traditional Chinese medicine group,30cases of each group.30cases in western medicine group were treated with western routine treatment.30cases in traditional Chinese medicine group were treated with western routine treatment and combined of the injection of salvia tetramethylpyrazine.The injection of salvia tetramethylpyrazine were used10ml per day, with Injection of5%Glucose250ml configuration,treated for2weeks.Detecting outside the peripheral venous blood of NSE,BNP levels and record the GCS (Glasgow coma score) score of all patients on the first day,the third day, the seventh day and the fourteenth day in hospital,while record the GOS score one month later,comparing the efficacy of the two groups.In addition,select another30healthy volunteers (normal group) test the NSE,BNP levels,the results were compared with traumatic brain injury.Results:1.The NSE level in the western medicine group and traditional Chinese medicine group were both increased on the first day after admission compared with the normal group (P<0.01);While the BNP level of western medicine group and traditional Chinese medicine group were both decreased compared with the normal group (P<0.01),the differences have statistics significance.2.The difference of NSE level in the western medicine group on the first day and the third day after admission both have no statistically significant compared with the traditional Chinese medicine group(P>0.05);The difference on the seventh day and the fourteenth day after admission both have statistics significance,the NSE level of traditional Chinese medicine group was lower than the western medicine group(P<0.05). 3.The differences of BNP level in the western medicine group on the first day,the third day and the seventh day after admission have no statistically significant compared with the traditional Chinese medicine group(P>0.05).The differences on the fourteenth day after admission have statistics significance(P<0.05),the BNP level of traditional Chinese medicine group was higher than the western medicine group.4.The differences of GOS score in the western medicine group on the first day and the third day after admission both have no statistically significant compared with the traditional Chinese medicine group(P>0.05).The differences on the seventh day and the fourteenth day after admission have statistics significance(P<0.05),the GOS score of traditional Chinese medicine group was higher than the western medicine group.5.The differences of GOS score in the western medicine group one month later after admission have statistics significance compared with the traditional Chinese medicine group(P<0.05),the prognosis of traditional Chinese medicine group is better than the the western medicine group.6.The serious of brain injury was negative correlated with the concentration of NSE (r=-0.867),the difference have statistics significance(P<0.05), the higher concentration of NSE indicate the heavier brain injury level.The prognosis of craniocerebral injury patients have negative correlation with concentration of NSE(r=-0.794),the difference have statistics significance(P<0.05),the higher concentration of NSE indicate the bad prognosis.7.The serious of brain injury was positively correlated with the concentration of BNP(r=0.806), have statistics significance(P<0.05), the lower concentration of BNP indicate more severe of traumatic brain injury.The concentration of BNP was positively correlated with the prognosis of craniocerebral injury patients(r=0.711), have statistics significance(P<0.05), the lower concentration of BNP indicate bad prognosis.Conclusion:1. The injection of salvia tetramethylpyrazine can get better efficacy and better prognosis when treated the patients of craniocerebral injury compared with the western routine treatment.The injection of salvia tetramethylpyrazine can effectively decrease the concentration of NSE and improve the concentration of BNP in the treatment of brain injury patients.2.The concentration of NSE had increased and the concentration of BNP had decreased in the craniocerebral injury patients compared with the normal people,and the concentration of NSE and BNP was closely correlated with the serious of brain injury and prognosis,the higher concentration of NSE and the lower concentration of BNP indicate more serious of traumatic brain injury and bad prognosis.Testing the concentration of NSE and BNP may indirectly reflect the severity of brain injury and can serve as one of the prognostic indicator in brain injury.
