| Objective To observe the effect of compound prescription salvia injection treat on hyponatremia after acute cervical spinal cord injury(ACSCI), explore compound prescription salvia injection on the prevention and treatment of hyponatremia after cervical SCI combine with clinical.Methods Part1:forty-five female SD rats,the average weight of289±9g,were selected and randomly divided into A,B,C group. This cross-section neck six-level posterior spinal cord, stomach fistula catheterization rat model was used. Normal food was given group A before surgery; group B and A were fed with TP (60ml/day). Before operation, the blood samples were collected. After surgery, the blood samples were collected every24hours,which were analyzed by the electrolyte analyzer.And the urine samples were collected every24hours.The daily average urinary sodium concentration was checked by the electrolyte analyzer.At the4th day, we put the rats to death by CO2and observe ordinary circumstances of the locations with surgery.Part2:fifty patients admitted to the Orthopedic Department of The175Hospital of PLA between2010and2011, which divided into the MP group (group A) and experimental group (Group B). the patients were admitted within24hours after injury. Group A:25cases,22were male and3were female, with a mean age of49.5years(range15to82years);The injury segment:C4in4,C5in3,C6in5,C7in6,C5-6in3,C6-7in2,and other2.Frankel classification:Grade-A6cases,Grade-B4cases,Grade-C7cases,Grade-D6cases,Grade-E2cases.Group B:25cases,20were male and5were female, with a mean age of48.3years(range22to69years);The injury segment:C3in2,C4in4,C5in5,C6in4,C5-6in6,C6-7in3,and other1.Frankel classification: Grade-A7cases,Grade-B6cases,Grade-C5cases,Grade-D5cases,Grade-E2cases. All patients were measurement chlorine of blood. Results Part1:there were no changes in weight,behavior and activity after feeding with TP before surgery. At the1st day after surgery,11rats were excluded because of death. Compared to normal diet, the SNa was not changed much by TP feeding. There is no significant difference. At the4th day after surgery, the SNa concentration loss or change was observed in the three groups. The decreases of SNa concentration in the three groups were shown significant difference.The rats in group C developed hyponatremia significant lighter than group A and B.The urine sodium concentration was collected daily after surgery. And the daily average urinary sodium concentration were increased since the3rd day after surgery. Compared to group C, group A and B rats have excessive urinary sodium loss. At the4th day, the rats were put to death by CO2after blood and urine samples were collected.Autopsies were done to shown that there was no TP or urine leakage and infection of the gastric fistula or Urethral catheter. In the lesion area of cervical spinal cord of rats, the complete ruptures of the spinal cord were observed and hematomases were formation in the interspaces of lesion spinal cords. Part2:two group of age and the extent of damage have not difference. Discovered through the technique latter2week blood sodium examination:B group of hyponatremia formation rate is lower than A group obviously. Compared group A to group B, the hyponatremia time of group B obviously later than group A, and the SNa obviously better than group A.Conclusion Combination of clinical and experimental studies, early large number of Compound Prescription Salvia Injection have a significant effected on hyponatremia after spinal cord injury, also lead to lower incidence rate, later apparent。 Compound Prescription Salvia Injection not only improve hyponatremia,but also produce microenvironment for spinal cord injury. |
