Study On The Effect Of Wei Chang An Pill In Ulcerative Colitis

Objective:Ulcerative colitis(UC), belonged to inflammatory bowel disease, is a non-specific inflammatory disease where pathological changes primarily attack the mucosa and the sub-mucosa of rectum and colon. The major clinical symptoms of UC include abdominal pain, diarrhea, tenesmus, hemafecia, and inordinately constitutional symptom. It occurs persistence or relapsing, and the disease severity is diverse. In western countries, UC occurs common. It has been classified as one of intractable disease by World Health Organization. For the past few years, with the change of lifestyles and the advancement of medical technology, morbidity and diagnostic rate of UC have been seeing a uptrend, more common in adults. For the time being, the pathogenesis, not clear and definite, is believed to relate to heredity, immunity, infection, environment and psychiatric factors by modern medicine closely. For clinical treatment, there are four types:aminosalicylic acid preparations, glucocorricoid, immunosuppressants and biological agent, lacking of safety and tolerance.For Traditional Chinese Medical, UC belongs to abdominal pain, dysentery, diarrhoea, et al. It is caused by six exogenous pathogenic, improper diet, emotional disturbance, spleen-stomach deficiency, kidney-yang deficiency and subsequent bowel dysfunction. Spleen deficiency and damp stagnation, consistent, are the most important base factors. Acute stage of UC is aways result in dampness-heat. So it should be treated by clearing heat and promoting diuresis, tonifying spleen and promoting the circulation of qi. Wei Chang An Pill can eliminate dampness with aromatics, promot the circulation of qi and tonify spleen. It has been proved to have anti-diarrhea and disinfect effect by multi-target. The research study focuses on UC. By UC rats model, we observe the effect of different ingredients on inflammation and mucosal barrier to explore the diverse targets on non-specific inflammatory bowel disease.Methods: 1. UC rats model was established by trinitrobenzenesulfonic acid (TNBS)/ethanol.2. Groups with different concentrations of Wei Chang An were established. Through test the changes of general status such as disease activity index (DAI), colon mucosa damage index (CMDI) and pathology, we try to explain the effect of Wei Chang An Pill to UC rats model.3. The level of TNF-α、IL-6、IL-10and the expression of COX-2were detected to discuss the adjusting effect of Wei Chang An on inflammation.4. By examining the secretion of mucin and the level of EGF in colon, we try to show the protection for mucosal barrier.Results:1. The UC rats model was induced triumphantly. The DAI and CMDI of UC model significantly increased compared to the normal (P<0.01). The ulcer were observed in pathology by general microscope.2. The DAI of high-dose and medium-dose were significantly lower than that of UC model (P<0.01); the CMDI of high-dose was significantly lower than that of UC model (P<0.01). By general microscope, the ulcer area and the quantity of inflammatory cells reduced obviously.3. High-dose and medium-dose significantly degraded the level of TNF-a and IL-6(P<0.05), increased the level of IL-10(P<0.01), and decreased the expression of COX-2.4. High-dose significantly improve the secretion of mucin and the level of EGF (P<0.01).Conclusions:The UC rats model was induced by TNBS/50%ethanol triumphantly. By comparing the level of DAI, CMDI, cytokine, and the secretion of mucin in mucosal barrier, we prove that:by the way of degrading the level of TNF-αand IL-6, increasing the level of IL-10and decreasing the expression of COX-2, Wei Chang An can inhibit mediators of inflammation, relieve the inflammatory response; by the way of increasing the level of EGF and improving the secretion of mucin, it can promote reparation of the colonic mucosa to protect the mucosal barrier. Adjusting the expression of cytokine and secretion of mucin is one pathway where Wei Chang An can effectively improve symptom and colon mucosa damage index.
