Objective: To compare the clinical effect of Rest guiding plate I-bar and Aker claspon free-end partial dentures. Methods: Case analysis method was use to make a returnvisit of122patients (125dentures) with free end defect wearing casting frame removablepartial dentures, of two groups sixty-three patients (sixty-three dentures) with RPI andfifty-nine patients (sixty-two dentures) with Aker clasp. One the one hand, subjectiveevaluation is being done to obtain information through a questionnaire about dentureappearance, degree of adaptation, degree of comfort and food impaction. One the otherhand, data collected through oral examination on abutment teeth loosing, base seal,retention and masticatory efficiency, and divided into two groups according to the types ofretainer. Results:The subjective evaluation of the denture appearance, degree ofadaptation, degree of comfort and food impaction etc of two groups was not significantlydifferent, and it was not statistically significant(P>0.05).Clinical examinations provedthat RPI was better than Aker clasp in some aspects which includes abutment teeth loosing,base seal, retention and masticatory efficiency(P<0.05).Conclusions: RPI has moreadvantages over Aker clasp on free-end partial dentures on certain aspects, e.g. reducingthe damage on abutment and periodontal tissue, reducing denture stability, improvingretention and masticatory efficiency. Therefore, adopting Rest guiding Plate I-bar design issuggested for patients with free-end defect.
