Clinical Effects Comparison Between DHS And FHR For Treatment Of Femoral Intertrochanteric Fracture On Elderly Patients

Objective: To compare the clinical effects and prognosis between dynamic hipscrew and femoral head replacement treatment for Tronzo-Evans type Ⅲ~Ⅳ intertr-ochanteric fracture, and to explore the reliable methods of internal fixation for theunstable fracture. Methods: From2006.7to2011.4,89elderly patients with Tronzo-Evans type Ⅲ~Ⅳ intertrochanteric fracture were selectedand analyzed Retrospe-ctively. The average age was79.7years (range,70~89years), all the patients wereclosed Tronzo-Evans type Ⅲ~Ⅳ intertrochantericfracture.The follow-up time was from7months to30months, average15months. Amongthem54patients (24malesand30females) in DHS group and35patients (16males and19females) in FHR group. surgery time, intraoperative blood transfusion, the time to leave bed, posto-perative complications, hip joint function (Harris) after1years’ surgery in bothgrou-ps were observed. Results: In the DHS group and FHR group, surgery time were (90.09±5.79) min and (94.71±7.27)min, intraoperative blood transfusion were (351.48±86.45) ml and(383.43±101.60) ml, the time to leave bed were (51.72±5.44)d and (10.20±2.19) d, There were4cases of infection (pulmonary,urinary),3case-s of DVT,3cases of cerebral infarction1case of plate breakage,3cases of screw out,2cases of coxavara after surgery in DHS group.There were2cases of pulmonary infection,1case of stress ulcer,1case of DVT after surgery in FHR group. the scores of Harris were (72.07±1.48) scores in DHS group and (85.31±2.36)sc-ores in FHR group. Conclusion: Because of many virtues, such as earlier perioda-ctivity out of bed, low incidence of complications, satisfaction of functional recove-ry, FHR is a reasonable preference for unstable intertrochanteric hip fracture in el-der patients,and the key point is to master theindication and rational operation.
