Clinical Research On The Effect Of Ailing Granule On Peripheral Blood T Cells Subsets And Th1/Th2Cells In HIV/AIDS Patients

AIDS (acquired immunodeficiency syndrome) is an immune deficiency disease which is appeared mainly with defects of T lymphocyte immune function caused by the human immunodeficiency virus infection. When the immune system destructed by HIV, people lost the ability of resisting infection and resulted in death. AIDS is one of the most infectious diseases which bring harm to human beings nearly30years. In China AIDS epidemic were estimated by the combination of China’s ministry of health, the United Nations AIDS program (UNAIDS) and the world health organization (WHO).At the end of2011, it is estimated that there are780000HIV/AIDS surviors.AIDS patients are154000; The crowd infection rate was0.058%(0.046%～0.070%).The interaction of HIV and the host’s immune system is the key factor of the AIDS. The sign of HIV progress is the progressive reducement of CD4+T lymphocyte number and the damagement of CD4+T lymphocyte function,which is an important performance of the immune system damagement. CD4+T lymphocyte function has been taken into consideration much more than before, especially the immune function evaluation, which is one of the hot topics in the study of HIV/AIDS prevention and control when the patients are treated by antiviral remedy. The imbalance of Th1and Th2cells is one of the main factors on the CD4+T lymphocyte function damage.A study found that people with HIV/AIDS whose T lymphocyte subsets gradually drift from dominant with Th1cells to Th2cells.Imbalance in Th1/Th2and AIDS disease progress are related closely.Objective:Guang’an’men hospital engaged the reserch on the traditional Chinese medicine for AIDS clinical and experimental, years of study found that Ailing granule intervention to HIV/AIDS patients could promote patients immune reconstruction, alleviate the clinical symptoms, improve the quality of life in a certain extent. But how to promote the role of immune reconstruction through traditional Chinese medicine, whether can improve CD4+T lymphocyte number and to improve its function, and whether it adjust Th1/Th2balance of the relationship between the cell type.This study makes a12month clinical observation to explore the influence of Chinese medicine of Ailing granule to HIV/AIDS patients peripheral blood T cells subsets and Th1/Th2balance. There are four purposes as follows:NO.1The difference of Th1/Th2cells between HIV/AIDS patients and healthy. NO.2The change of T cells subsets, Th1/Th2cells and viral load on AIDS patiens after Ailing granule treatment. NO.3The change of T cells subsets, Th1/Th2cells and viral load on AIDS patiens after pure HAART treatment and co-therapy of Ailing granule and HAART.Methods:This research focuses on20cases of HIV infections,40cases of AIDS patients. Traditional Chinese medicine diagnosis is qi deficiency and blood stasis.20cases of HIV infections were treated with Ailing granule for a period of12months.40cases of AIDS patients were randomly divided into two groups, the western medicine group and integrated Chinese medicine and Western medicine group, seperately treated with placebo cooperate with HAART or Ailing granule cooperate with HAART, lasting for a12month period.Make the detection about patients’T cells subsets, viral load and cytokine regularly.Visit patients and fill in the registration form of clinical observation regularly.Record the clinical symptoms and signs score and Karnovsky score.At the same time recruit18cases of healthy volunteers as control group,with cytokine detection. Use SPSS17.0to make statistical analysis about the indicators, and summarize the research results.Results:In the process of the experiment,2cases in the integrated Chinese medicine and Western medicine group failed off, so the analysis includs18cases of healthy volunteers,20cases of HIV infections,20cases of AIDS patients treated with HAART and18cases of AIDS patients treated with Ailing granule and HAART, the results are as follows:CD4+T lymphocytes:the quantity of CD4+T lymphocyte in both of the western medicine group and integrated Chinese medicine and Western medicine group treatment group increased with no Statistical significance (P>0.05). There were no Statistical significance between the two group (P>0.05). The quantity of CD4+T lymphocyte in Chinese medicine treatment group was stability in a certain range which is no Statistical significance (P>0.05)The viral load:The viral load of the western medicine group inecreased with no Statistical significance (P>0.05).The viral load of the integrated Chinese medicine and Western medicine treatment group decreased with no Statistical significance (P>0.05). There were no Statistical significance between the two groups (P>0.05). The viral load in the Chinese medicine treatment group inecreased with no Statistical significance (P>0.05)IFN-y:HIV/AIDS patients’IFN-y secretion levels were significantly lower than healthy controls, with a statistically significant (P<0.05). The level of IFN-y in western medicine treatment group was increased obviously after6and12month(P<0.05). The level of IFN-y in the Combine traditional and western medicine treatment group inecreased obviously after12month (P<0.05). There were no Statistical significance between the two group (P>0.05).In Chinese medicine treatment group the level decreased with no Statistical significance (P>0.05)IL-2:HIV/AIDS patients’IL-2secretion levels were significantly lower than healthy controls, with a statistically significant (P<0.05). The level of IL-2in western medicine treatment group was increased slightly after6and12month(P>0.05). The level of IL-2in the integrated Chinese medicine and Western medicine treatment group inecreased obviously after12month (P<0.05). There is no Statistical significance between the two groups (P>0.05).In Chinese medicine treatment group the level increased obviously after12month (P<0.05).IL-4:AIDS patients’IL-4secretion levels were significantly higher than healthy control groups, which is significant (P<0.05). The level of IL-4in western medicine treatment group was at a stable level after the treatment(P>0.05). The level of IL-4in the Combine traditional and western medicine treatment group inecreased slightly(P>0.05). There were no Statistical significance between the two groups (P>0.05).In Chinese medicine treatment group the level increased slightly after the treatment (P>0.05).IL-6:HIV/AIDS patients’IL-6secretion levels were significantly lower than healthy control group,which is statistically significant (P<0.05). The level of IL-6in western medicine treatment group increased slightly after the treatment(P>0.05). The level of IL-6in the Combine traditional and western medicine treatment group inecreased obviously after6and12month(P<0.05). There were no Statistical significance between the two groups (P>0.05).In Chinese medicine treatment group the level increased sustainingly after6and12months (P<0.05).Conclusion:this study lasts a12month period of clinical and laboratory index observation.After the evaluation analysis, preliminary results are as follows:1. Ailing granul can do a certain extent help to maintain the quantity of CD4+T lymphocytes in HIV infectors.2.Ailing granul can do a certain extent help to remain the viral load in a stable level in HIV infectors.3.It is coincide with previous studies that there is potential advantages and encouraging signs to improve IL-2by Ailing granul treatment.4.Ailing granul can do a certain extent help to delay the drift of Thl cells to Th2cell in HIV infectors.
