Objective:(1)To obtaine detailed anatomical data of transverse process of third lumbar vertebra.(2)To design and making the instrasegment fixation instrument for transverse process of third lumbar vertebra (TPOTLV).(3)Analyzing the finite elementmethod(FEM) and evaluating the ability of the new device to restore the fracture stability, for further clinical application provides theory basis.Methods:(1)20sets of adult intact lumbar dry specimens were elected and numbered, the third lumbar transverse proeesses (tTLTP) as the research object, the parameters of20lumbar transverse proeesses, including length, height, thiekness of tTLTP and the angles of inclincation of tTLTP and horizontal section were measured by method of imageology and anatomy.(2)By the datas from measuring20cases of dried adult lumbar spine specimens, to design a instrasegment fixation instrument for third lumba transverse process fracture.Random selecting ten samples of the third lumbar, Saw the the third lumbar left transverse proeessehorizontal to make fracture models.,divide them to two random groups,,each group was tested by non-destructive and destructive axial compression and torsion experiment with data recorded.(3)Random selecting10sets of L3dry specimens, divide them to two random groups, sawing the left of specimens to make fracture models,using the holder fixed. the left of specimens is the experimental group,the right of specimens L3is the control group. Each group was tested by non-destructive and destructive axial compression and torsion experiment with data recorded.(4)The data of healthy men’s the lumbar spine was got by CT, E-feature Biomedical Modeler were used to establish geometry model.First for accurate three-dimensional segmentation and reconstruction model, and then the adaptive finite element mesh dynamic biomechanics delimit from three-dimensional entity model can generate high quality directly into the finite element model of the lumbar spine. The finite element model will import ANSYS software, and determined the model of loading boundary conditions and the loading method, the simulation three main direction load loading, understand the third lumbar at normal model and the stress fracture model.Results:(1)Measured value of the third lumbar transverse process related areas are as follows:the length of the lumbar spine at (L) is21.90±2.26mm; At the height of the (H) is8.26±1.78mm; The thickness of the sudden (T) is3.64±0.42mm; The axial plane of the third lumbar transverse proeess inclination was2.95°±0.686°; The coronal angle was10.5°±1.23°, and there was no significant difference between CT measure and anatomic measure.(2)According to the measuring results, the designed the third lumbar instrasegment fixation instrument, this device including pedicle screw fixation and the instrasegment fixation instrument for transverse process of third lumbar vertebra (TPOTLV), of which the TPOTLV in shape and the third lumbar transverse process anatomical structure correspond. Drawing and choose304stainless steel made in the TPOTLV.(3)Biomechanical testing results are as follows:the maximum compressure and twisting intensity of the experimental group were94.97±2.26N,1.90±0.13N-m;which in the control group were98.08±2.19N,1.58±0.14N-m.(4)Three-dimensional finite element analysis results are as follows:the lumbar spine at normal side and fracture side in loading at load size displacement no obvious difference; The fracture simulation results that the normal side and the equivalent stress fracture without obvious difference.Conclusion:The third lumbar transverse process fracture fixed with the third lumbar instrasegment fixation instrument can meet at the biomechanical needs, for clinical treatment at the fracture to provide some experimental basis.
