Background Intervertebral disc degeneration and endplate abnormalities may be thecause of discogenic low back pain, extensive literature has reported that inflammatorycytokines play in lumbar degenerative diseases and their important role in studies ofinflammatory cytokines in the lumbar endplate degenerativepathological roles of andrelationship between the change in more accurate and effective treatment measures, toguide clinicians to intervene.Objective To investigate the expression of TNF-α and MMP-3in Modic Changes onMRI.Provide a theoretical basis for the treatment of degenerative low back pain.Methods Experimental group, the First Affiliated Hospital of Anhui MedicalUniversity,,July1,2010to June31,2011due to lumbar degeneration with lumbarspinal instability in hospital patients in our hospital lumbar interbody fusion20cases,age42-68years, mean55-year-old, previously had a history of low back pain,including MRI, ModicI10cases, L4/53patients, at L5/S17cases; lI10cases, L4/52cases, L5/S1cases, due ModicIII rare, this experiment does not collect ModicIIItype of change in endplate specimens, the control group disc endplate specimens wereobtained from the same period due to the lumbar spine trauma patients in our hospitallumbar interbody fusion, a total of10cases, age23to42years old, average33yearsold, patients with previous low back pain history of the disc near the cartilageendplate region vertebral MRI signal changes had no Modic change.The applicationof immunohistochemical staining method of determination of the disc end plate ofTNF-a, MMP-3expression. Results1.Modic the I-type10cases of patients in the experimental group, TNF-α are positive,the expression rate of45.94±15.57%, Modic II-type10cases of patients,9cases ofTNF-alpha-positive, the expression rate of29.11±16.19%, the positive rate ofexperimental group was95.00%.Control group of10cases only two cases of TNF-αpositive rate of20.00%.The experimental group of TNF-α expression wassignificantly higher (P <0.05).2.Modic type I10cases of patients in the experimental group, MMP-3are positive,the expression rate of50.83±16.21%, Modic II-type10cases of patients, eight casesof MMP-3positive, the expression rate of24.09±15.30%, the positive rate ofexperimental group was90.00%.The control group of10cases, only one case ofMMP-3positive rate of10.00%.The experimental group MMP-3expression level wassignificantly higher (P <0.05).3.Experimental group of MMP-3, TNF-a expression in Modic Type I endplate weresignificantly higher than ModicII type (P <0.05).4.TNF-αand MMP-3expression was not correlated.Conclusion The expression of TNF-αand MMP-3may promote vertebral endplatesdegeneretion;TNF-αand MMP-3expression was not correlated.The immunoreactivecells rate of TNF-α and MMP-3is associated with Modic Changes on MRI....
