Purpose To explore the effect to clinical neurological defects, ADL and Quality of Life by Tuina with conventional rehabilitation exercises. The purpose of this article is to analyze and to provide a reliable and effective method which is Tuina with rehabilitation exercise on the stroke patients.Methods The investigation was conducted by means of case control study, including100cases of acknowledged stroke-patients who came from the hospital of combining traditional Chinese and western medicine, Shandong Province, during the period2010,6-2011,3, and randomly divided them into rehabilitation with Tuina group and controlled group. It was including85cases of Brain Infarction and15cases of Brain Hemorrhage in this study. One hundred cases of patients were divided into two groups,50cases into the rehabilitation with tuina group and50cases, the control one without tuina. Also they were no prominent difference in sex, age, pathological nature, side of paralysis and severity of illness at admission. These two groups of patients had all accepted the conventional rehabilitation therapy of neurology at acute phase in encephalopathy department. When the rehabilitation group was given neurological conventional rehabilitation therapy, as soon as their disease was controlled they would receive early bedside rehabilitation therapy (in1month’s time), then they were transferred to the rehabilitation center receiving rehabilitation therapy (in3months’time), till the middle and latter period of therapy(in3months-6months’time), it was continued in community (such as family ward). The rehabilitation therapy took the form of physical therapy (PT) and Occupational Therapy (OT), in accordance with the patients’brunnstrom rating. The Contrast group’s therapy was no tuina therapy; it received only conventional rehabilitation exercise. The rehabilitation effect was evaluated in neurological impairment measuring scale (1995) at the forth national academic conference on cerebrovascular disease (Neurological deficit score, NDS), which was done with Neurological defect degree for neurological function, ADL in modified Barthel index, and SF-36for the investigation of quality of life on post-stroke patients. So the degree of neurological defect was reflected, the abilities of life activity was marked Barthel index assessment scale, which were taken at the beginning time and the3th month,6th month respectively to analyze and appraise on the apoplexy patients at the end of the course.Main Results1when the observation group (rehabilitation training group) and controlled group were formed, the score of Neurological defect was respectively35.66and35.56. The difference between them bears no statistic significance (P>0.05). The results showed the score of observation group was26.52and22.38at the time point of3months and6months. Compared with the beginning, the score of observation group gradually declines significantly9.14and13.28after the treatment of Tuina with rehabilitation exercise in3months and6months. The score of control group was32.9and28.56, the score of control group declines2.66and7.0after the treatment of rehabilitation exercise only in the same time compared with the initial stage. There is a significant difference between the two groups (P<0.01), also there is a significant difference at the differ time of checking in the observation group (P<0.01) whereas no significant difference in the control group (P>0.05).2There was no statistical difference of the ability score of daily life at admission in the two groups (P>0.05). The scores of BI were37.22and35.28initially. Compared with the two groups after the treatment of3and6months, the group of the rehabilitation with tuina was higher than that control group. The results showed the scores of observation group were67.48and85.90while were35.88and62.04of the control group at the same time (P<0.01). There was a significant difference after the treatment of3months and6months than the time of the onset on the two groups(P<0.01).3Comparing of the dimensions of Quality of life:in the SF-36performance test, changes in quality of life were revealed clearly with SF-36health survey received treatment lasted3and6months. There was a significant difference on the Physical function compared with the time of the treatment of3and6months in the observation group (P<0.01). The score of the observation group in the dimension of Vitality (VT) and Mental health (MH) was significant difference comparing with control groups (P<0.05,P<0.01).There was no statistics difference on the dimensions of Role-emotional (RE) and General Health (GH) after the time of3and6months between the two groups (P>0.05).The three dimensions of Role-physical (RP), Bodily Pain (BP) and Social functioning (SF) was no significant difference at the time of3months (P>0.05) while were better after6months received the treatment compared with the two groups.At the same groups, there was a significant difference on the Physical Functioning (PF) after3and6months than the onset of the observation group(P<0.01).There were significant difference on the dimensions of Role-emotional (RE) and Vitality (VT) after3and6months compared with the beginning (P<0.05,P<0.01).Compared with the dimension of Mental Health (MH), General health (GH) and Role-physical (RP) showed significant difference on the two groups after treatment respectively at the time of3and6months (P<0.05),but no statistical significance at the treatment in3and6months on the dimensions of Bodily Pain (BP) and Social Functioning (SF)(P>0.05).Conclusion:1.There is a significant effect on the degree of Neurological defect, to raise the ability of life activity, and to improve the quality of life on the post-stroke patients by Tuina with rehabilitation exercise compared with conventional rehabilitation therapy. So Early standardized rehabilitation training with Tuina can improve and enhance the patients’recovery effectively on apoplexy patients.2.It has an effect of quality of life on the stroke patients. So hope that it can improve their physical functioning, the same that it will improve their vitality,mental health,social functioning etc.It has improved the patients’quality of life for the stroke patients.
