Objective: To observe the possibility of direct anastomosis of contralateral C7transferred through prespinal route with Target nevers. Meanwhile if we can find aroute to avoid the neurovascular injury and this route is more simple,safe and shortenthan prespinal routeMethods:The bilateral brachial plexus were exposed on20sides of10cadavericspecimens of adult. The C7nerve root was sectioned at the junction site of trunk anddivision,and then dissected proximally to the foramina.the following data wererecorded: The length of bilateral C7nerve,The length of the anterior and posteriordivision of C7,The distance from the roof of C7to the upper trunk at the affectedside through prespinal route and a subcutaneous tunnel on the anterior surface of theneck.In order to measure C7nerve terminals which got through the Spinousclearance,We had dissected and unfolded lower third region of the neck。and in themeantime,we had measured the distance from the roof of C7to upper trunk and lowertrunk at the affected side through posterior route。Results:The length of C7was (58.62±8.70)mm,The length of anterior andposterior division of C7were(70.03±10.79)mm and(65.15±9.11)mm,The distancefrom the roof of C7to the upper trunk at the affected side through subcutaneoustunnel,prespinal route and posterior route was(98.18±10.18)mm,32.10±11.49)mm and(43.22.±5.96) mm;The distance from the roof of C7to the lowertrunk at the affected side through prespinal route subcutaneous tunnel and posteriorroute was(107.14±9.88)mm and (37.28±10.01)mm and(54.31±7.80)mm,there were statistical significance among them(P<0.01)In the posteriorapproaches of cervical spine,we do not find any important nerve and vessel。Conclusion:from the point of anatomy, the contralateral C7transfer throughprespinal route can shorten obviously the length of nerve graft than subcutaneous tunnel. But it does not reduce the quantity of anastomotic stoma. Therefore,there isno possibility of direct anastomosis of contralateral C7transferred through prespinalroute with upper trunk and lower trunk.Although the neurovascular injury wasavoided in the posterior approaches of cervical spine,it does not reduce the length ofnerve graft。...
