| Objective To evaluation the clinical application value ofretroperitoneal retroperitoneal LRN in T2RCC.Methods Summary the clinical data of68cases underwent RN for T2（7cm<tumor<10cm） renal cell carcinoma in the first affiliated hospital ofChongqing medical university between2006and2009.30cases byretroperitoneal laparoscopic radical nephrectomy and36cases by ORN.Thepatients’ demographic data of the two groups is similar. All cases werepathologically confirmed as clear cell carcinoma. We analysis perioperativesurgical outcomes between the two groups,include the operativetime,theblood loss, the time of recovery of bowel function,perioperativecomplications,the time in hospital,the time of early ambulation, the rate ofanalgesic.compared long-term oncologic outcomes between the two groups.Results Compare with ORN group, the operation time ofretroperitoneal retroperitoneal laparoscopic radical surgical group beextended （185±29min，p<0.05）.the operative blood loss（280±210ml，p<0.05）,the time of recovery of bowel function （27.1±7.7h，p<0.05）, the time in hospital（9.0±2.0d, p<0.05）,the time of early ambulation（3.0±1.0d,p<0.05）, the rate of analgesic（13.3%, p<0.05） were significant better inretroperitoneal LRN group. complications in retroperitoneal LRN groupwere intraoperative bleeding2cases,wound infection1case, pneumonia1cases, intestinal obstruction2cases; complications in open surgical groupwere intraoperative bleeding2cases, wound infection3cases, pneumonia1cases, intestinal obstruction2cases.The complication rate of two groupswere20.0%、22.0%respectively,no significance. Follow-up was from36months to72months in the two groups. Cancer recurrence rate and5-yearoverall survival rate was13.3%and78.0%respectively in the laparoscopicgroup.There were no significant differences in the5-year overall survivalrate and cancer recurrence rate.Conclusion we suggest that retroperitoneal retroperitoneal LRN for themanagement of T2RCC is feasible and efficacious. |
