MRI Volume Measurement Of Mammillary Body And Initial Application In Cognitive Impairment Disease

Abstract/Summary:
Objective:The symmetry of side, evolutional trend with age increase and gender difference of mammillary body (MB) volume were observed and analyzed in order to represent the MB distribution of normal adults. Besides, the MB volume of mild cognitive impairment (MCI) and Alzheimer disease (AD) patients were studied, so as to investigate the MB change in cognitive dysfunction diseases and evaluate its clinical application.Methods:50normal adults.25normal control subjects.25MCI patients and25AD patients were recruited in this study. GE HD-X3.0T MR scanner was used to obtain MR images of normal adults and normal control subjects. The MR images of the MCI and AD patients were downloaded from ADN1. Measurement of MB volume used ITK-SNAP Version2.0.0software, based on3D high-resolution T1VVI anatomy imagings and repeated twice. Then the average volume was normalized by whole brain volume. SPSS19.0statistical package was used to analysis of the MB standard volume indexes and the methods included paired samples t-test, independent samples t-test and single factor analysis of variance.Results:In normal adults, the averages of MB standard volume and total standard volume were100.2±11.2mm3and200.4±20.1mnr3and there were non statistical differences of side, age or gender. The compare inside each group of MCI or AD showed that the right MB standard volume (MCI92.2±19.4mm3. AD79.1±14.5mm3) were larger than the left (MCI82.9±17.2mm3. AD71.2±15.6mm3) and the differences were significant (tmci=3.920. tAD=3.422. p <0.05). Moreover, it showed non statistical difference of gender. The compare among the normal control. MCI and AD showed statistical differences (F standard solume=35.965. F total=21.610. p <0.05). The MB volume change trend: normal control (standard volume101.3±11.0mm3total202.5±19.7mm3)> MCI (standard volume87.6±18.8mm3, total175.2±34.7mm3)> AD (standard volume75.2±15.4mm3, total150.3±27.8mm3). And all of the differences were significant (p <0.05). Furthermore, side respective compare among three groups also showed statistical differences (Fright=14.782. Fleft=23.366. p <0.05). The MB volume change trend: normal control (right102.5±10.4mm3, left100.0±11.7mm3)> MCI (right92.2±19.4mm3, left82.9±17.2mm3)> AD (right79.1±14.5mm3. left71.2±15.6mm3). And all of the differences were significant (p <0.05).Conclusion:1.The distributional features of MB volume in normal adults: bilateral symmetry, non significant gender difference and non obvious atrophy with age increase.2.MB volume decreased in cognitive dysfunction disease, such as MCI and AD. The atrophy of MB started in early AD and the progress was consistent. MB volume was an sensitive but poor specific index in early AD.3.In MCI and AD patients, left MB atrophy was more apparent than right, whieh tiped on non symmetry of MB atrophy in cognitive dysfunction disease.
