| ObjectiveTo observe the changes of serum adiponectin in T2DM complicated with nonalcoholic fatty liver(NAFLD) patients,and to investigate the relationship between adiponectin levels and clinical features.MethodsAll of the85persons were divided into the control group,T2DM group and T2DM complicated with NAFLD group.Surveys of all the subjects were about life style and medical history.The patients were measured for height,weight,waist circumference.hip circumference,blood pressure of all the subjects.Laboratory tests included blood glucose, lipid profile, liver function, renal function, islet β-cell function under their fasting condition. BMI,WHR.Homeostasis model assessment of insulin resistance index and other related index were also calculated.The serum samples were collected and stored in-80℃. The serum adiponectin and HMW adiponectin were tested by Western blotting and ELISA.Results1Compared with patients in control and T2DM group, the mean values levels of BMI,WHR.ALT,AST,TC were higher in T2DM complicated with NAFLD group (P <0.05);2Adiponectin and HMW adiponectin and H/T were lower in T2DM group than control group, but FINS. HOMA-IR were higher (P<0.05);Compared with patients in control and T2DM group,the mean levels of adiponectin and HMW adiponectin and H/T were lower, FINSã€HOMA-IR were higher in T2DM complicated with NAFLD group (P<0.05);3Western botting results show that adiponectin can be detected by it. Adiponectin molecular weight was about30KD,HMW adiponectin molecular weight greater than250KD,MMW adiponectin molecular weight was about150KD. 4APN,HMW APN levels were the highest in control group and lowest in T2DM complicated with NAFLD group. Much greater amount of adiponectin in females compared to male in T2DM group; Much greater amount of HM W adiponectin in females comparedto male in all groups;5Pearson correlation analysis showed that serum adiponectin was negative correlate to WHR,AST,TG,LDL-C,FPG(P<0.01), ALT,TC.FINS, HOMA-IR (P<0.05);HWM adiponectin was negative correlated to WHR,FPG,HOMA-IR(P<0.05).Adiponectin and HMW adiponectin were positive correlated to HDL-C(P<0.01);Stepwise multiple regression analysis showed that WHR,TG,HOMA-IR wer e independently associated with adiponectin (P<0.05);WHR was independe ntly associated with HMW adiponectin (P<0.01), HOMA-IR was independ ently associated with HMW adiponectin (P<0.05)6Logistic results of adiponectin and T2DM complicated with cardiovascular show that the decline of adiponectin and elevate of HOMA-IR were risk factors of cardiovascular;Conclusions1The T2DM patients complicated with NAFLD were most obesity, visceral adiposity obviously and hepatic function injury;2The serum adiponectin, HMW adiponectin and H/T levels were lower. The serum adiponectin level was lowest and insulin resistance more serious in T2DM patients complicated with NAFLD;3The serum adiponectin, HMW adiponectin levels were correlated toWHR.HOMA-IR;4The decrease of adiponectin was the independent factor of the T2DM complicated with cardiovascular. |
