Objective: To explore whether different ways of handing the pancreatic stump canaffect the incidence of the pancreatic fistula. Methods: The clinical data of181casesbetween june2006and july2011underwent pancreatoduodenectomy were analyedretrospectively. In the operation, the types of anastomosis we always use arepancreatic-enteric anastomosis and pancreatic-gastric anastomosis.pancreatic-entericanastomsis includes pancreaticojejunal anastomoses by means of Duct-to-mucosa,traditional invaginated pancreaticoenterostomy and modified end to end invaginatedpancreaticojejunostomy.Results: Pancreatic fistula was found in5cases who accecptedpancreatic-gastric anastomosis.pancreatic-enteric anastomsis and29cases who accecptedpancreatic-enteric anastomosis.In the29cases,25cases acceped traditional invaginatedpancreaticojejunostomy,4cases accepted pancreaticojejunal anastomoses by means ofDuct-to-mucosa,people who accepted modified end to end invaginatedpancreaticojejunostomy havenot found any pancreatic fistula.Conclusion: Different typesof handing the pancreatic stump may affect the incidence of the pancreatic fistula. In orderto decrease the incidence of pancreatic fistula, we should intoduce the advantages ofdifferent types of handing the pancreatic stump,at the same time we should individualizethe anastomosis. |
