| Background and objectives:Irritable bowel syndrome(IBS) is one of the most common functional gastrointestinal disorder in primary care. IBS is defined as recurrent abdominal pain or discomfort,associated with a change in the frequency of stool and/or onset associated with a change in form or appearance of stool. The prevenlance rate is vey high,and increasing year by year, it really does harm to the patients,however the precise etiology of IBS is still unknown.Abnormal gastrointestinal molity、visceral hypersensitivity、Low-grade inflammation、Intestinal excessive gas production、gut microflora of environmental change、 genetic、and psychologic and spiritual factors were associated with it,in recent years,many studies reported low-grade inflammation and immune activion were found in intestinal mucosa among the patients suffered from IBS.Immune activion is not only confined to the intestinal mucosa,but also reflected in the activity of mononuclear cells secrete cytokines、the basic level of cytokines increased highly.the unbalance between the pro-inflammatory cytokines and the anti-inflammatory cytokine may play a part role in the abnormal immune response,this study examined Interleukin(IL)-1β、IL-6、IL-10expression in serum and sigmoid colon mucosa of IBS patients and the healthy persons to confirm the unbalance between pro-inflammatory cytokines and the anti-inflammatory cytokine, investigate the relationship between it and the clinical symptoms,prepare the ground for the fouther research.Methods:1.According to the RomeⅢ criteria,40patients were recruited randomly from gastroenterology clinics in the third of xiangya hospital,at the same time10healthy persons recruited from the medical centre were setted as the control,analysis the genenal information of them.2.Collection the peripheral boold4ml,selected the serum after the centrifugation,measure the level of the IL-1β、IL-6、IL-10by Enzyme Linked-Immuno-Sorbent Assay(ELISA),and the analysis outcome with t-test.3. Colonic mucosa were taken by routine colonoscopy,and IL-1β、IL-6、 IL-10in these mucosa were examined with Immunohistochemistry(IHC). And then analysis the outcome with Fisher probabilities in2X2table.4. Evaluate the40IBS patients with questionnaires, including:irritable bowel syndrome-quality of life questionnaire(IBS-QOL), irritable bowel syndrome-symptom severity scales(IBS-SSS), and preliminary study the correlation with cytokinesResults:1.pro-inflammatory cytokinesIL-1β、IL-6serum level were22.58±3.48pg/ml、42.03±4.65pg/ml,which were higher than control group markedly (p<0.05), anti-inflammatory IL-10serum level were33.97±4.92pg/ml, which was lower than control,differed no significantly,(p>0.05),and there were no differences among the three subtypes.2.pro-inflammatory cytokinesIL-1β、IL-6positive expression frequency in colonic mucosa were95%、92.5%,which were higher than control group significantly(p<0.05),anti-inflammatory IL-10positive expression frequency in colonic mucosa in IBS patient was52.5%,there were no significantly difference with the control group (p>0.05). and there were no difference among the three subtypes.3.The mean scores of patients tested by IBS-SSS、IBS-QOL were285±90.5、63.2±25.83,and they had the negative correlation,the Pearson correlation r=-0.649(p<0.05).4.Respectively set the IBS-SSS、IBS-QOL as the dependent variable in multiple stepwise regression analysis to get the possible influencing factors were:the ratio of IL-6/IL-10and the ratio of IL-6/IL-10、the subtypes of cases、Course of disease.Conclusion:In this research,the expression level of IL-6、L-1β in serum and colonic mucosa increased in IBS patients,however, IL-10level had no significant difference, those outcomes may hinted that the unbanlance between anti-inflammatory and pro-inflammatory may play an important role in the pathogenesis of IBS.2.In multiple stepwise regression analysis, the ratio of IL-6/IL-10may play a role of IBS-SSS, at the same time,the ratio of IL-6/IL-10、the subtypes of cases、Course of disease may impact the the quality of life of ibs patients. |
