Abstract/Summary:
Objective:To observe the clinical effects and the changes of serum activity of Platelet Activating Factor (PAF) after massive cerebral infaction who undertook mild therapeutic hypothermia(MHT), and to discusses the Clinic-al Implications in patients, and to explore the protective effect of mild therapeu-tic hypothermia to brain.Methods:47Patients with acute massive cerebral infarction, who should be insulted within48hours, were randomly divided into hypothermia therapy group and conventional treatment group. The hypothermia therapy group had24patients. The conventional treatment group had23patients. The conventio-naltreatment group accepted conventional treatment. The hypothermia therapy group accepted conventional treatment and hypothermia therapy which last48hours. The neural function deficiency scores and the serum activity of PAF were compared before and48hours after treatment. We used the U.S.Nationallnstitut-es of Health Stroke Scale (NIHSS) to obtain the Neural Function Deficiency Scores, and used the method of Enzymelinked Immunosorbent Assay to detect-ed the serum activity of PAF. At the meantime, we also detected the serum ac-tivity of PAF from healthy persons as normal control group.Results:1.Compared to normal control group, the PAF activity in hypothermia group and conventional treatment group were significantly increased before and48hours after treatment (p<0.05)2.The PAF activity of hypothermia group was169.5±13.04pg/ml, while the conventional treatment group was171.3±10.10pg/ml. Difference between the two groups were not statistically significant (p>0.05). After48hours trea-tment, The PAF activity of hypothermia group was significantly decreased to151.9±10.83pg/ml, and the PAF activity of convertional group was decreasd to163.7±12.77pg/ml (p<0.05)3. The NIHSS score of convertional treatment group and hypothermia group had little difference (p>0.05). After48hours, the NIHSS score of convertional treatment group decreased to29.6±6.71, while the hypothermia group decrease to24.0±4.83(p<0.05)Conclusions: 1. Compared to normal control group, the activity of serum PAF in patients with acute massive cerebral infarction was significantly increased.2. Local Mild Therapeutic Hypothermia within48hours could obviously lower the activity of serum PAF in Patients with acute massive cerebral infarction, and could obviously improve the neurological function.3.Selective head mild therapeutic hypothermia through the way of decrees-ing the activity of serum PAF has the neuroprotection, which maybe is one of Therapeutic hypothermia’s mechanism.
