| Objective: Traditional Chinese medicine was used to nourishing kidney andstrengthing spleen，replenishing qi and blood among patients with unexplained recurrentspontaneous abortion（URSA）。 Through observation of clinical curative effect，we caninvestigate the influence of nourishing kidney and strengthing spleen on CD4~+CD25~+regulatory T cells in peripheral blood in patients with URSA and study the m echanism。And then find an effective way to treat URSA patients。Methods:90patients who meet inclusion criteria were enrolled in this study, and theywere allocated into three groups according to the random number table，includingTraditional Chinese medicine（TCM）group（n=30）, immunotherapy group（n=30） andthe control group（n=30）。The TCM group were treated with Self-developed TCMprescription to nourish kidney and strength spleen, once a day over a course of21days.After3courses, patients were encouraged to get pregnant in natural way. Theimmunotherapy group were treated with the initiative immunotherapy, once per course of21days, after3courses, patients were encouraged to get pregnant in natural way. Thecontrol group got no treatment but only life guidance for2months and relaxation beforepregnant. The three groups each got conventional therapy with progesterone and HCGafter the certain of pregnancy for3months. The percentage of CD4＋CD25+regulatory Tcells in peripheral blood，successful pregnant rates,abortion rates, the anti-HLA Ⅰ/Ⅱantibodies positive rates and TCM symptom score were compared before and aftertreatments among the three groups. Analyze the relationship between pregnancy outcome and anti-HLA Ⅰ/Ⅱ antibodies in the three groups. The number of CD4＋CD25~+regulatory T cells were also compared before and after treatments with different pregnancyoutcomes.Results: There was no statistical difference among the three groups in age，abortiontimes and gestational weeks when abortion（p>0.05）. There was no statistical differenceamong the three groups before treatment. But the percentage of CD4~+CD25+regulatoryT cells in peripheral blood were significantly increased in TCM group and immunotherapygroup after treatment compared with the control group, and also significantly increasedcompared with that before treatment（P<0.05）。The TCM symptom score in TCM groupwas significantly decreased compared with the other two groups and that before treatment（P<0.05）. The successful pregnant rates and the anti-HLA Ⅰ/Ⅱantibodies positive ratesin TCM group and immunotherapy group were significantly increased compared with thecontrol group（P<0.05）. The percentage of CD4＋CD25~+regulatory T cells in successfulpregnant women was significantly higher than that in pregnant loss women(P<0.05).Andthere was a positive correlation between successful pregnant rates and the anti-HLAⅠ/Ⅱantibodies positive rates（P<0.05）.Conclusions: Traditional Chinese medicine nourishing kidney and strengthing spleencan improve the spleen and kidney deficiency syndrome, enhance the percentage of CD4＋and CD25~+regulatory T cells in peripheral blood and the anti-HLAⅠ/Ⅱantibodiespositive rate, accordingly,can increase immune function and pregnancy rates and thendecrease abortion rate.It is an effective therapy for patients with URSA and has theadvantages of easy operation and practical.Therefore, it deserved to be widely used. |
