To survey the status of Helicobacter pylori (HP) and its genotype infection incholangiocarcinoma, to analyze the relationship between clinical and pathologicalparameters and survival prognosis, to investigate the prognostic value of theirinfection combined with clinical data and follow-up data.MethodsThe infection of HP in80cholangiocarcinoma samples and30control sampleswere detected by polymerase chain reaction(PCR), with all positive results tofurther test the expression of Cag A and three subtypes of Vac A sla, m1and m2byPCR, Combined with clinical data, pathological parameters and follow-up data toanalyze the results.Results1. Chi-square test showed that the detection rate of HP (+) in cholangiocarcinomagroup is71.25%, which is higher than that in control group20%, the difference isof significance in statistics science (χ~2=23.418,P<0.05);2. The positive cases of CagA and VacA sla,m1, m2in cholangiocarcinoma groupis30,40,5,43. The main genotype of HP (+) in cholangiocarcinoma is VacAsla/m2.3. There is no relationship between HP infections and staging of tumor(χ~2=1.204,P=0.548). The infection of HP in cholangiocarcinoma was correlatedwith the location of the tumor (χ~2=27.580,P<0.05). Multivariate logisticanalysis showed that the possibility of cholangiocarcinoma in patients HP (+) isover10times that form HP (-)(OR=10.531).Cox regression analyses showedthat the infection of HP(HR=8.105,P=0.032), the staging of TNM(stage Ⅱ /ⅢHR=9.141,P=0.040,stage ⅣHR=29.071,P=0.040) and the situationof surgical (HR=9.531,P=0.015) are all independent prognostic factors ofcholangiocarcinoma, and Life table analyses showed the infection of HP affectthe survival time in cholangiocarcinoma postoperative(u=10.074,P=0.002), andthe median survival time is less than HP(-) by7.25months.Conclusion1. These results suggested that the infections of helicobacter pylori may exist incholangiocarcinoma,.2. the major genotype of HP in cholangiocarcinoma is VacA sla/m2,.3. HP infection has a relationship with the occurrence of cholangiocarcinoma, andagainst survival prognosis of cholangiocarcinoma postoperative, which maycontribute physiological and pathological effects to the occurrence anddevelopment in cholangiocarcinoma. The related mechanism needs furtherresearch.
