Objective:In recent years,more and more research on homocysteine and vasculardisease were done.It Is generally believed that homocysteine is an injury amino acid.Inflammation, oxidative stress, immune response process cause pathological damage byhomocysteine which has a very important relationship with atherosclerosis. Diabeticmacroangiopathy is a major cause of death disability to diabetic patients. There is a greatsignificance to search for predictors of early lesions of macroangiopathy. Cystatin C,asa cysteine inhibitors,is generally believed that the reaction of the glomerular filtrationrate indicators. In diabetic vascular disease it is still less.To study clinical significanceand relationship among levels of Hcy and Cysc in serum vascular complications and therelevance of serum Hcy and CysC,FPG,HbAlC,TC,TG,SBP,DBP,LDL,HOMR-IRand others in elderly T2DM.Methods71elder patients with T2DM were divided into2groups:36elderly T2DMwithout vascular complications;35elderly T2DM with vascular complications.22healthy volunteers were chosen as the healthy control group. The objects of study fromAnhui Provincial Hospital from February2010to June2011in Department of Geriatrics,Endocrinology, hospitalized patients and healthy individuals. Every group three monthsbefore the study were not used to the impact of folic acid, vitamin B12and other drugsthat affect HCY metabolism.Levels of Hcy,Cysc were respectively determined in serumin91objects of study.At the same time,general condition and biochemical indicator ofobject of study were collected,as FPG,TC,TG,LDL,FINS and so on.RESULTS①.The levels of Hcy,Cysc of patients from the T2DM with vascularcomplications group(C group) were significantly higher than of those from the T2DM without vascular complications group(B group). It shows that the level of serum Hcyhave important relationship with diabetic macroangiopathy.The Levels of Hcy,Cysc Bgroup control were significantly higher than of those from the healthy control group(p<0.05).②With diabetes big vascular lesions as the dependent variable,Multiplestepwise regression analysis showed that the level of Hcy,Cysc maybe the effect factorsof Hcy in T2DM.It showed dominance ratio of Hcy(6.319),95%CI(1.658,19.249))and Cysc(3.872),95%CI(1.156,12.746.Conclusion Serum Hcy levels are elevated in patients with diabetic macroangiopathy is,compared with the T2DM without vascular complications group, a statistically significantdifference. Serum Hcy and Cysc level was positively correlated.Hcy and Cysc can beused as a valuable reference target in the diagnosis and monitoring of vascularcomplications in elderly T2DM.
