Clinical Significance Of The Expression Of Survivin In Benign And Malignant Gastric Ulcer Tissue

AIM:1.To study150cases of paraffin-embedded specimens from different gastric ulcermucosa of the inpatients in the5years, and to investigate the expression of Survivin andits clinical significance in non-precancerous lesion, precancerous lesion and malignantgastric ulcer.2.To contrast the expression of Survivin in the150cases of gastriculcerative mucosa with the expression of CEA and CA199,and to study the clinicalapplication of Survivin which may become to a specific tumor mark in malignantgastric ulcer.3.Deeper research was made on precancerous gastric ulcer, and tounderstand the imperceptible and significance of the expression of Survivin in theprocession of benign gastric ulcer becoming to malignant gastric ulcer.METHODS:1.Retrospective studyed150gastric ulcerative lesion inpatients between2006-2010at the derpartment of gastroenterology, the second hospital of Hefei.Clinicalinformation and examine results were protocoled indetail and arranged, including48cases of non-precancerous lesion,49cases of precancerous lesion and53cases ofmalignant gastric cancer. The S-P immunohistochemical method was used to detectthese cases.2.AB-PAS and Alcanblue (PH1.0) stained method was used to differentiatethe colonic and small intestine intestinal metaplasia,then record the results of expressionof Survivin.RESULTS:1.The positive expression rates of Survivin,is24.5%,62.5%,73.6%,respectively in non-precancerous gastric ulcer,precancerous lesion and malignant gastric ulcer,and is higher in malignant gastric ulcer and precancerous gastric lesion than innon-precancerous gastric ulcer(P<0.01),is higher in intestinal metaplasia gastric ulcerthan in dysplasia gastric cancer(P<0.05); That in precancerous lesion gastric ulcer andin malignant gastric ulcer, the result is not statistically significant（P>0.01）.2.Thepositive expression rates of Survivin in colonic intestinal metaplasia is24.5%, higherthan that in small intestine intestinal metaplasia (P<0.001).3.The expression of CEA inthese three groups cases are0%,2.1%and34.0%, respectively; The expression ofCA199in these three groups cases are0%,6.3%and39.6%, respectively. Now, weknow that the expression of Survivin in malignant gastric ulcer is more sensitive thanCEA and CA199(P<0.01).CONCLUSIONS:1.The positive expression of Survivin rises followed the change ofcell heterotype in gastric ulcer lesion.It tells us that there is certain clinical significancefor the difference of the expression of Survivin in benign and malignant gastriculcer.2.To detect the expression of Survivin in precancerous gastric ulcer,and followddynamically, Survivin may reflected in clinical significance of evolvement of gastriculcer and identification between benign and malignant gastric ulcer.
