ObjectiveEndometrial carcinoma is one of common malignant tumor of the female reproductive tract and account for20%to30%of the malignant tumors of female reproductive tract. Its incidence is on the rise in recent years and the age of onset largely covers the younger demography.15%to25%are diagnosed at an advanced stage and the clinical treatment is very challenging. Early diagnosis is helpful to improve the survival and quality of life.Tumor stroma plays an important role in tumor growth, certain proteoglycans present in the extracellular matrix are closely associated with tumorigenesis, spread of tumor and metastasis. Biglycan(BGN), a component of extracellular matrix, is a member of class I small leucine-rich proteoglycans(SLRPs). Recent studies have shown that Biglycan is closely related with the development of various tumors. The overexpression of biglycan in endometrial cancer has been studied by some scholars. However, the biglycan expression in serum and the supernatant of peritoneal washings has not been reported untill now. Therefore we adopt method of ELISA to detect the content of biglycan in the supernatant of blood and peritoneal lavage fluid collected from the patients with endometrial cancer to explore its relationship with the clinicopathological aspect and clinical significance, to understand the relations between biglycan and the occurrence and development of endometrial cancer further.Methods Enzyme-linked immunosorbent assay was used to detect the expression of Biglycan in serum and supernatant of peritoneal washings samples from13cases with normal ednometrial,10cases with atypical endometrial hyperplasia,57cases with endometrial cancer and then its relationship with clinicopathological features and clinical significances were discussed. Serum and peritoneal lavage fluid from57cases with endometrial cancer were collected in between June2011to February2012from the department of Gynecology in Qilu hospital of Shandong University. None of the patients had any other medical conditions and none had received any auxiliary preoperative treatment like chemotherapy, radiotherapy and hormonal therapy. All of them were tested for CA125in the blood samples and histopathologic method was employed for the diagnosis after the surgery in each case; also complete clinicopathological information about each case was compiled precisely.Results1. The concentrations of biglycan in serum and supernate of peritoneal washings of endometrial carcinoma patients were significantly higher than that of normal control group and atypical hyperplasia group (P<0.05).2. The expression of biglycan in serum of patients with endometrial cancer was associated with FIGO stages (P<0.05), histologic grade (P<0.05), lymph node metastasis (P<0.05) and abdominal rinses cytology(P<0.05) with a significant difference in each case. Alternatively the concentration of the biglycan in the peritoneal lavage fluid specimen of the same patients showed no significant correlation with age, FIGO stage, and histological grade, histological type, depth of muscular infiltration and lymph node metastasis (P>0.05).3. Biglycan content in serum of endometrial atypical hyperplasia and endometrial cancer patients were higher than in peritoneal washings, but showed no significant difference (P>0.05), but there existed a significant correlation between expression of biglycan in serum and in ascites(r=0.401, P=0.002).4. The area of biglycan under the ROC curves in serum、peritoneal lavage fluid is0.819and0.805,and serum CA125is0.766. In our experiment, the sensitivity of CA125is38.46%to endometrial cancer of Stage I,its diagnosis sensitivity is49.12%.When the cut-off value of biglycan content in serum reach2.81ng/ml, in peritoneal washings reach2.78ng/ml, the diagnosis sensitivity is significantly higher than the CA125(P<0.05). The sensitivity for diagnosing endometrial cancer of Stage I is higher than the CA125. Three united detection, the sensitivity of diagnosis to endometrial cancer can achieve87.72%.ConclusionThe expression of biglycan in the peripheral blood、peritoneal lavage fluid rom endometrial carcinoma patients is related to occurrence and development of endometrial cancer, and it is expected to be a potential tumor marker which is conducive to early diagnosis of endometrial cancer needing to expand samples for further study.
