OBJECTIVE:To detect the selenium (Se) and zinc (Zn) level in serum and tissue of patients with nodular goiter and papillary thyroid carcinoma. To investigate the relationship between the two rare essential elements and the two diseases mentioned above. And to find new methods to prevent the two diseases.METHODS:The serum and tissue samples are collected from the patients who had thyroidectomy from October2011to January2012in Qilu Hospital of Shandong University. The healthy persons’serum samples are also collected as normal controls. The serum samples are divided into three groups:The papillary thyroid carcinoma(PTC) group, which included33cases,11males and22females, age from14to64, the nodular goiter group, which included68cases,15males and53females, age from21to79, and the normal control group, which included50cases,14males and36females, age from22to75. While the tissue samples are divided into four groups according to the result of histopathologic diagnosis.:The normal group,29cases, which is collected from the thyroid tissue1cm at least apart from the nodular, and the histopathologic diagnosis is nodular goiter. The nodular goiter group,45cases, which is collected from the nodular part, and the histopathologic diagnosis is nodular goiter. The paracarcinoma group,41cases, which is collected from the thyroid tissue1cm at least apart from the tumor part, and the histopathologic diagnosis is papillary thyroid carcinoma. And the PTC group,37cases, which is collected from the tumor part, and the histopathologic diagnosis is papillary thyroid carcinoma. Detect and compare the selenium and zinc levels of each group. The selenium is detected by the method of hydride generation atomic fluorescence spectrometry, and the zinc is detected by the method of flame atomic absorption spectrometery.RESULTS:1. The serum selenium level:the serum selenium level is86.61±23.95ug/L、78.43±28.81ug/L、63.31±26.04ug/L in normal control group, the nodular goiter group and the PTC group. There is no statistically significant difference between the normal control group and the nodular goiter group. However there is statistically difference between the normal control group, the nodular goiter group and the PTC group. The serum selenium level in PTC group is lower than the normal and goiter group.2. The tissue selenium level:the tissue selenium level from high to low is:the normal group> the nodular goiter group> paracarcinoma group> PTC group. There is statistically difference between the nodular, paracarcinoma, PTC group and the normal group. The tissue selenium level in nodular, paracarcinoma, PTC group is lower than the normal group.3. The serum zinc level:the serum zinc level is2.02±0.44ug/mL1.36±0.53ug/mL、1.29±0.45ug/mL in normal control group, the nodular goiter group and the PTC group. There is statistically difference between the nodular group, PTC group and the normal control group. The serum zinc level in nodular, PTC group is lower than the normal group. However there is no statistically significant difference between the nodular goiter group and the PTC group.4.The tissue zinc level:There is no statistically significant difference among the four groups.CONCLUSIONS:1. The serum and tissue selenium level in PTC group is significantly lower. The state of low selenium may take part in the pathogenic process of papillary thyroid carcinoma. It indicates that we can take selenium supplement to prevent papillary thyroid carcinoma.2. The tissue selenium level in nodular goiter group is lower, although the serum selenium level is normal. The low tissue selenium level have relationship with nodular goiter. 3. The serum zinc concentration of the nodular goiter group and the PTC group is lower. Low serum zinc level have relationship with the nodular goiter and the papillary thyroid carcinoma.
