Study On The Financial Share And Compensation Mechanism Of The Basic Public Health Services In Chongqing

9kinds of basic public health services have launched in2009, and topromote the basic public health services gradually equalization as one of thefive major tasks of the medical health system reform in China.According to the main conclusion of the project report of “Research onthe implementation strategy of basic medical and health system of WHO” in2008, discovered that the cause of the providing and operating mechanism isnot perfect in the basic public health services in the financial field mainlyinclude: First, as a result of vague of local policy and the imperfect of theFiscal Guarantee Mechanism, there is a serious shortage of the supply ofworker、money and material, local finance is unable to meet the needs of thebasic public health service demand of the local population. Second, as aresult of no investigation for scientific and effective demand before thelevels of public finance investment, some public financial investment failedto protect the key of demand of people’s livelihood of basic public healthservice, and the public finance expenditure is unreasonable and lowefficiency; Third, as a result of inadequate supervision of public finance funds, the results are lack of performance evaluation, and some basic publichealth service for urban and rural residents for free has not beeneffectively;Fourth, because there is no scientific evaluation mechanism andcompensation incentive mechanism, the working efficiency of the primaryhealth care personnel is low, and effect of differential; Fifth, due to the lackof divided power, project and proportional share of the financial investmentsecurity mode, all levels of basic public health service providers hard to clearthe scope of services and expenditure responsibility. Therefore, study on thefinancial share and compensation mechanism in Chongqing is verynecessary and meaningful.This paper analyzed the financial investment amount of the totalexpenditure on health and public health, and the financial share, thencompared with other countries, and found that the level of investment inhealth and public health in our country is far below the most countries in theworld, and at all levels of financial sharing, central investment ratio isrelatively low, the local governments face finance pressure. Then throughthe sampling investigation and analyzing the financial investment amount ofthe total expenditure on health, public health and basic public health servicesin Chongqing. Found that there are still some problems: First, the level of percapita funding is too low, the total investment is insufficient; second, thefinancial system and the transfer payment system is not perfect; third, largedifference between areas at financial investment; Fourth, there is a contradiction between supply and demand in rural areas, and large differencebetween urban and rural areas; Fifth, the performance evaluation system ofthe basic public health services is not perfect. To solve these problems, somesuggestions had been proposed: improve the financial system and thetransfer payment system; strengthen the government investment in the basicpublic health responsibility; improve the system of the basic public healthservice; strengthen the performance appraisal of the basic public health.
