The Experience Of Diagnosis And Treatmengt Of Pancreatic Duct Stone

Objective:To discuss the experience in the diagnosis and treatment of pancreatic duct stone(PDS).Methods:The28patients with PDS were treated at our hospital from2002to2011,which were analyzed retrospectively in terms of clinical characteristics, diagnosis and treatment.Results:Patients were all lack of specificity symptoms,epigastric pain was the clinical feature of the disease.There was no specific indicators to diagnosis the disease for laboratory testing,The diagnosis of all cases was confirmed by imaging examinations such as B uhrasonography,CT and MRCP.According to the results of imaging examination,PDS was classified into four types.6patients underwent conservative treatment.2patients of I type underwent Whipple procedure,18patients of II type underwent Pusetow procedure,1patient of III type underwent distal pancreatectomy and spleenectomy,l patient of IV type underwent subtotal pancreatectomy and Roux-en-Y pancreaticojejunostomy. All the cases were followed up from6months to18months.Among6cases with conservative treatment,the abdominal pain alleviated in5cases and unalleviated in lcases.Among22cases with surgical treatment,the abdominal pain disappeared in18cases,and other are alleviated in different degree.Conclusions:The patients of PDS were lack of specificity clinical manifestation.BUS was the first choice for diagnosis in addition to CT and ERCP for accuracy. Treatment should follow the principles of individualized therapy.According to the classification of PDS,we should take the corresponding operation method,the Pusetow procedure was the main operative method for PDS.
