Objective To evaluate the effects of carbamazepine(CBZ) and oxcarbazepine(OXC) monotherapy on vascular risk factors in adults with epilepsy. Methods This clinical trial was a prospective study,71newly diagnosed adultpatients with epilepsy were enrolled in this study as trail group, select CBZmonotherapy(CBZ treatment group,n=35) and OXC monotherapy(OXC treatment,n=36) according to the individual basic situation. Before treatment, body massindex(BMI),plasma TC, TG, LDL-C, HDL-C, LP(a),homocysteine(Hcy),C-reactiveprotein(CRP) and uric acid levels were measured in the trail group and compared themwith74controls;The indicators above-mentioned were mearsured again after6monthsmonotherapy, compared them with the indicators of trail group before initiation, andthen compared the indicators between two treatment group;Multple regression analysiswere adopted to detect the relationship between indicators above-mentioned and thefactors such as during, seizure types and doseResults There were no statistically significant differences in gender, age, height,BMI and average fat intake between trail group and control group. In tems of thecomparison of vascular risk factors:1.Before treatment,TC,TG,HDL-C, LDL-C,LP(a),Hcy,CRP and uric acid were not significantly different between trail group andcontrol group (P>0.05).2.In CBZ treatment group,TC,LDL-C and LP(a) weresignificantly increased after treatment when compared them with the indicators thatmeasured before treatment,Hcy and CRP were also significantly increased after CBZtreatment,no significant differences in uric acid were observed in CBZ treatmentgroup. In OXC treatment group, compared with the indicators that measured beforetreatment,Hcy levels were observed significantly increased after treatment, and therewere no significant differences in lipid parameters, CRP and uric acid aftertreatment.Compared to OXC treatment group,TC and CRP were significantly increasedin CBZ treatment group(p>0.05)3.Multiple stepwise regression analysis revealed thatthere was a significant positive correlation between gender and TC,there was also asignificant positive correlation between Hcy and dose of OXC.Conclusion The partial seizures itself did not influence the vascular risk factors,Both CBZ and OXC might influence the vascular risk factors.CBZ mightincrease the lipid parameters,HCY and CRP,OXC might increase the Hcy.Both of thetwo drugs did not influence the uric acid levels.CBZ have more influence onHcy,serum lipids and CRP than OXC.The HCY levels might increase with the dose ofOXC.
