Objective:To analyse the nipple areola complex(NAC) direction of cancerouscarcinogenesis regular pattern by detecting the protein expression of hTERT andRASSF1A which may be the cell proliferation or malignant transformation potentialrelated tumor markers,attempt to provide the evidence of molecular pathology in thedirection of NAC,to further improve the safety of breast-conserving operation.Methods: Mastectomy specimen from its NAC direction edge1cm,2cm,3cm ofparacarcinoma tissues were marked separately. All were fixed by10%formalin,andmade into paraffin block with conventional methed. Cut blockes in the middle with4μm thickness into three pieces standby, one was stained for HE, the other two werestained for hTERT and RASSF1A IHC (EliVision plus), respectively.Results:Immunohistochemical results show that hTERT positive expression graduallydecreasing trend from the cut edge organization1cm,2cm,3cm in the direction ofNAC(P<0.05),RASSF1A positive expression gradually enhanced trend from the cutedge organization1cm,2cm,3cm in the direction of NAC(P<0.05), and bothabnormally express range within2cm. The infiltrating range in the direction of NAC isirrelated to pathological types, tumor size and positive expressions of ER/PR, C-cerb-2,and lymph node group(P>0.05),which is related to histologic classification andage(P<0.05).Conclutions: Cells in molecular level changes in the direction of NAC were mainlyexisted within2cm,it is recommended that2cm is safe extension for breast-conservingoperation. The positive expression of hTERT and negative expression of RASSF1Anearby primary tumor may be precancerous changes, it ma ybe reduce the localrecurrence possible by surgical resection,and it is necessary to choice individualtreatment for breast cancer with different clinicopathologic characteristics.
