Objective:By determining the expression of survivin and XIAP in the renal clear cell carcinoma(RCCC), renal para-carcinoma tissues and normal renal tissues to investigate theeffect and significance of occurrence and development in the renal cell carcinoma(RCC); and the correlating with biological behavior of RCC; and to provide a basisfor clinical diagnosis, treatment and prognosis for RCC.Methods:The expression of survivin and XIAP in55cases of RCCC,15cases in the renal para-carcinoma tissues and10cases in the normal renal tissues were determined byimmunohistochemistry, then analyzed the correlation of the expression and thepathological grade, clinical stage, metastasis or not and other clinicopathologicalbehaviors.Results:1. The cytoplasmic staining of survivin could be seen clearly. The positive rate ofsurvivin in RCCC was63.6%(35/55). The positive rate in the renal para-carcinomatissues was6.7%(1/15). The cytoplasmic staining was not observed in the normalrenal tissues. The positive rate in RCCC group was significantly higher than the rate inthe control group (P<0.01), that was to say, the expression in the RCCC of survivinwas significantly higher than in the renal para-carcinoma tissues and normal renaltissues.The positive rate of male was66.6%(26/39), the positive rate of female was56.3%(9/16), it meant, the expression of survivin in RCCC had no significant differencewith sex (P>0.05). The positive rate of below50years was60.9%(14/23), thepositive rate of over50years was65.6%(21/32), the expression of survivin in RCCC had no significant difference with age (P>0.05). The positive rates of survivin in welldifferentiated group, moderately differentiated group and poorly differentiated groupwere47.4%(9/19),60.0%(12/20) and87.5%(14/16); according to pathological graderespectively, with the increasing of the grade, the intensity of positive expression ofsurvivin increased, there was statistical difference (P<0.01). The positive rates ofsurvivin in stage â… /â…¡ and stage â…¢/â…£ were51.4%(18/35) and85.0%(17/20);according to clinical stage respectively, with the increasing of the stage, the intensityof positive expression increased; there were statistical difference (P<0.01). Thepositive rate in non-metastatic RCCC and metastatic RCCC were54.8%(23/42) and92.3%(12/13); it showed that, the intensity of positive expression of survivin wassignificant difference between these two groups (P<0.01).2. The positive staining of XIAP was in cytoplasm, occasionally in the nucleus. Thepositive rate of XIAP in RCCC was69.1%(38/55). The positive rate in the renalpara-carcinoma tissues was26.7%(4/15). The positive rate in the normal renal tissueswas20.0%(2/10). The positive rate in RCCC group was significantly higher than therate in the control group (P<0.01), that was to say, the expression in the RCCC ofXIAP was significantly higher than in the renal para-carcinoma tissues and normalrenal tissues. The positive rate of male was74.4%(29/39), the positive rate of femalewas56.3%(9/16), it meant, the expression of XIAP in RCCC had no significantdifference with sex (P>0.05). The positive rate of below50years was65.2%(15/23),the positive rate of over50years was71.9%(23/32), the expression of XIAP inRCCC had no significant difference with age (P>0.05). The positive rates of XIAP inwell differentiated group, moderately differentiated group and poorly differentiatedgroup were52.6%(10/19),70.0%(14/20) and87.5%(14/16); according topathological grade respectively, with the increasing of the grade, the intensity ofpositive expression of XIAP increased, there was statistical difference (P<0.01). Thepositive rates of XIAP in stage â… /â…¡ and stage â…¢/â…£ were57.1%(20/35) and90.0%(18/20); according to clinical stage respectively, with the increasing of thestage, the intensity of positive expression increased; there were statistical difference(P<0.01). The positive rate in non-metastatic RCCC and metastatic RCCC were 54.8%(23/42) and92.3%(12/13); it showed that, the intensity of positive expressionof XIAP was significant difference between these two groups (P<0.01).3. In55cases of RCCC, the positive expression of survivin were35cases, the positiveexpression of XIAP was38cases. There were7cases that show positive expression ofsurvivin and negative expression for XIAP, and10cases that show negativeexpression for survivin and positive expression of XIAP. There were28cases thatshow positive expression of survivin and XIAP, and10cases that show negativeexpression of survivin and XIAP. The expression of survivin was positively correlatedwith the expression of XIAP in RCCC (r=0.312, P=0.022).Conclusions:1. The expression of survivin and XIAP in RCCC was significantly higher than thosewere in the renal para-carcinoma tissues and normal renal tissues, that suggestingsurvivin and XIAP may play an important role in the occurrence and developmentabout RCC. And it may be the potential therapeutic target for therapy of advancedRCC.2. The higher expression of survivin and XIAP in RCCC was correlated withhistological grade, stage and metastasis. It suggested that survivin and XIAP may beassociated with aggressive behavior of RCC. It will be helpful in developing afollow-up plan and assessing prognosis.3. There was a positive significant correlation of survivin and XIAP expression inRCCC. It seems that survivin and XIAP play a synergy synergistic role in theformation and development of RCC.
