Part Ⅰ The study of related influence factors in urolithiasis patientsObjective To analyze and explore the related influence factors in urolithiasis patients.Methods Date from April,2010to May2011, data from214patients admitted to thedepartment of Urology and192normal heathy volunteers at the same time period, Changhaihospital, were retrospectively reviewed. Blood and urine were taken from the patient the nextmorning after admission. Serum levels of electrolytes, lipid and urine Ca, urine PH werechecked by automatic biochemistry analyzer. The data were analized by software SPSS17.0.Results The level of serum Ca, P and uric acid in urolithiasis group were significantly higherthan normal control group, there was significant differences between two groups (P<0.01),the level of serum CO2CP was significantly lower than normal control group, there wassignificant differences between two groups (P<0.05); The level of serum TG and ApoE weresignificantly higher than normal control group, there was significant differences between twogroup patients (P<0.01), the level of serum HDL-C was significantly lower than normalcontrol group, there was significant differences between two groups (P<0.05), compared tothe level of serum TC, LDL-C, ApoA and ApoB between two groups, there were nosignificant differences between two group patients (P>0.05); The multiple logistic regressionanalysis showed that hyperlipidemia and family histroy were entered variable model, whoseOR value and95%CI were1.321(1.145,1.467),2.234(1.768~2.814) respectively.Conclusions There was difference in serum electrolyte and lipid levels between urolithiasisgroup and normal control group, and hyperlipidemia may be a risk factor in urolithiasisformation.PartⅡ The study of relationship between ApoE gene polymorphism andurolithiasis incidenceObjective To elucidate the relationship between ApoE gene polymerphism and urolithiasisMethods Collected the blood sample from154urolithiasis patients and111normal controlsin Changhai hospital during Apirl,2010to December,2011, detected single nucleotidepolymorphism of rs429358and s7412locus in ApoE gene, statistically, analyzed andcompared the frequency distribution of two SNP gene types and allelic genes betweenurolithiasis and normal control group. Results In urolithiasis group, the frequency of rs429358locus TT, TC, and CC gene typewere83.1%,14.9%and2.0%, respectively, the frequency of T and C allelic gene were90.6%and9.4%, respectively; the frequency of rs429358locus TT, TC, and CC gene type in normalcontrol group were79.3%,20.7%and0%, respectively, the frequency of T and C allelic genewere89.6%and10.4%, respectively; there was no significant differences between urolithiasisand normal control group in the frequency distribution of each rs429358locus gene type andallelic gene (P>0.05). The frequency of rs429358locus TT, TC, and CC gene type inurolithiasis group were0,12.3%and87.7%, respectively, the frequency of T and C allelicgene were6.2%and93.8%, respectively; the frequency of rs429358locus TT, TC, and CCgene type in normal control group were2.7%,21.6%and75.7%, respectively, the frequencyof T and C allelic gene were13.5%and86.5%, respectively; there was significant differencesbetween the two group in frequency distribution of each gene type (χ2=8.710,P=0.01), thefrequency of CC gene type in urolithiasis group was significantly higher than that in normalcontrol group, TC gene type as a reference, CC gene type have increased significantly the riskof urolithiasis incidence (OR=2.030,95%CI:1.049-3.930,P=0.033); there was significantdifferences between the two group in frequency distribution of each allelic gene (χ2=8.294,P=0.004), the frequency of C gene type in urolithiasis group was significantly higher than thatin normal control group, C allelic gene as a reference, T allelic gene have increasedsignificantly the risk of urolithiasis incidence (OR=2.337,95%CI:1.301-4.343,P=0.004);In the patients with urolithiasis family history and hyperlipidemia, the risk of urolithiasisincidence in CC gene type was8.000,5.200times than TC gene type (OR=8.000,95%CI:1.061-60.324,P=0.030,OR=5.200,95%CI:1.181-22.891,P=0.022), there was no significantdifference between TC and CC gene type in the patients with no urolithiasis family historyand hyperlipidemia; there was no significant difference between T and C allelic type in thepatients with or no urolithiasis family history and hyperlipidemia.Conlusions The C allelic gene in ApoE gene rs7412was susceptible locus to urolithiasis, CCgene type was risk factor of urolithiasis incidence, in addition, the genotype had interactioneffect with metabolic factors and family history, increased the risk of urolithiasis.
