ObjectivesThe main aim of this study is to investigate and analysis the clinical characteristics of diarrhea-predominant irritable bowel syndrome in Traditional Chinese Medicine(TCM) and its distribution of TCM syndromes, in order to deepen the understanding of various types, and as far as possible enrich IBS-D’s contents in TCM, so as to make it more consistent with the distribution of modern IBS in TCM, and make it more closer together with clincal, in order to increase the accuracy of diagnosis, improve the curative effect and convenient for standardizing and extending the types of syndromes; and summarizes the main type of TCM syndrome, so that the future development of targeted therapy for clinical research, to provide guidance and help.MethodsUsing observational research methods, designing a unified questionnaire which include the general information of patients, case history, symptoms and signs, tongue demonstration pulse tracings and electronic colonoscopy result, then collecting patients in Guangdong hospital of TCM who is diagnosed on IBS-D according to the inclusion standard and exclusive standard of IBS. Finally, establishing a database for deseriptive statistics and inferential analysis.Result1. The study included115patients, with male patients accounted for46.96%, women accounted for53.04%, male-female ratio1:1.13, the average duration of4.93±5.45years; the average age of37.98±12.47years, among30-39years old patients are the most common;the highest prevalence ration of university degree or above (53.1%), followed by high school, junior high school, primary school and below. Among predisposing factors, diet is the most common, followed by mood, fatigue, cold, and presented a diversity.2. Stool traits change accounted for97.39%of all patients. Sense of defecation not the flu accounted for44.3%, sense of defecation urgency accounted for44.3%, and anal burning sensation is relatively rare, with only4.35%.3. In distribution of the syndrome types,58cases of the type of stagnation of liver-QI with deficiency of the spleen(50.43%),45cases of the type of splenasthenic hygrosis(39.13%), and12cases of asdthenic splenonephro-yang (10.44%), and not found the type of splenogastric hygropyrexia(0.00%). In elusion types, heat accountes for15.65%, and accountes for9.57%of damp-heat, blood stasis accountes for7.83%, renal deficiency accountes for6.96%, damp accountes for3.46%, in which the type of stagnation of liver-QI with deficiency of the spleen has elusion types most easily. The distribution of IBS-D’s major types of syndromes in sex, culture degree have no significant differences (P <0.05), there is significant difference in age(x2=6.443, P=0.035<0.05), which mainly embodied in the groups of under30years old and over50years old.Conclusion1.The distribution of the IBS-D syndrome is basic consistent with the generalization of "traditional Chinese Medicine Consensus on Irritable bowel syndrome" on the syndrome types, suggesting that the consensus opinion and clinical actual suits. This study found no the type of splenogastric hygropyrexia, which is considerd that the diagnostic criteria of consensus opinion on the spleen stomach damp heat syndrome may be too demanding.2. IBS-D syndrome distribution and patients’sex, cultural level are not related; while in the age there is certain difference, but after the analysis thinking it is related with the bodys’natural growth law, suggesting that in clinical syndrome differentiation process the symptoms of patients should be signed as the main reference, age is just one factor to consider.
