| Object To observe the safety and efficacy of gefitinib concurrent with individualizedthoracic radiotherapy (TRT) in patients with locally advanced lung adenocarcinoma.Toevaluate their quality of life, and to perform a subgroup analysis of EGFR mutations,hemoglobin concentration, performance status, gender and radiation dose in thecombination therapy. Methods Between November2009and March2011,37patientswith Stage IIIa/IIIb lung adenocarcinoma were enrolled in this study. Patients inconcomitant group were treated with gefitinib250mg oral daily concurrent withindividualized TRT. The other group was treated with TRT only.Then compaire theefficacy and toxicity of two different treatment for locally advanced lung adenocarcinomain patients. EGFR mutations,hemoglobin concentration,performance status, gender andradiation dose were inrolled into a subgroup analysis.Result For the combination groupcompared to the TRT group:RR was78.9%vs44.4%,(P=0.03);DCR was89.4%vs61.1%,(P=0.04);the1-year-control rate was82.3%vs54.2%,P<0.001;the1-year-surnivalrate was4.2%vs61.1%,PP<0.001;the2-year-surnival rate was42.1%vs22.2%;P=0.02;Median TTP was6.4months vs4.2months,P=0.009;Median PFS was8.9months vs6.7months,P=0.025;MST was23months(95%CI:21.59-24.41months)vs15months(95%CI:8.80-21.19个months),p=0.041;Gefitinib with concurrent thoracic therapygroup in the Global QOL,physical function,emotional function,cognitive function,social function and related symptoms such as cough,hemoptysis,chest pain,a significantimprovement in dyspnea,fatigue,anorexia and radiotherapy alone group was similaradverse effects can be tolerated. Multivariate analysis showed that the PS score andhemoglobin concentration affect the survival rate of the combined treatmentgroup.Conclusion Gefitinib with concurrent thoracic therapy for locally advanced lung adenocarcinoma tends to have better results than radiotherapy alone,and the toxity can betoletated. |
