Background:Cervical and Lumbar Disc Herniatio(nCDH/LDH)are common diseasesin Orthopaedic clinic. The quality of work and life is influenced greatly by the pain of thediseases. Conservative and operative treatment are two main methods while conservativetreatment has the disadvantage of low efficiency and long therapy period and operativetreatment has high expense to be hospitalized and high complications which lead todeclination from patients. Coblation Nucleoplasty(CN) is a kind of minimally invasivetreatment method which can coblate the disc on low temperature with the help of imageguide and decompress the nerve root which can release the symptom immediately. In ourarticle, we will compare the clinical results of the two methods and discuss the indicationsand contraindications, operative approach as well as prevention and treatment of compli-cations.Objective:Retrospective comparative study of clinical effects of Coblation Nucle-oplasty and conservative treatment in Cervical/Lumbar Disc Herniation.Methods:54patients were involved in our research in which24patients were treatedwith CN and30patients were treated with conservative method. All patients were followedup by1and3months. VAS(Visual Analogue Score) and SSR(Subjective SatisfactionRatings)were used to appraise the results of two methods. We also compare hospitalizationperiod and recovery time as well as complications. All the data were treated by statistics.Results: There were no difference between the two methods in age(p>0.05), sexualratio(p>0.05), VAS score(p>0.05) and the ratio of Cervical and Lumbar Disc Herniation(p>0.05) before treatment. There were significant difference between two groups inexcellent rates(VAS score: p<0.05, subjective satisfactory score p<0.05), averagehospitalization time(p<0.01)and average recovery period(p<0.01)after treatment. Thereis no complication in two groups and there was no statistical difference of complicationbetween two groups.Conclusions: There are three advantages for Coblation Nucleoplasty in Cervical andLumbar Disc Herniation compare with conservative treatment:1.It can release the pain quickly and has more reliable result in primary period.2. It can reduce the probability ofrecurrence after nucleus pulposus coblation and shrinking.3. It has the advantage of shorthospitalization time and recovery time. Diadvantages: It may cause comp lications because ofdifference ability of operative doctors; Patients and doctors are exposed to X ray; There willbe more medical expense compare with conservative method.
