Mdct And Painless Gastroscope In Liver Cirrhosis Stomach Esophagus Varicosity Diagnosis Andtreatment Of Analysis

Background and Purpose:To evaluate the diagnosis rate of multidetector CT and painless gastroscope in thediagnosis of esophageal-gastric varices caused by portal hypertensionMaterials and Methods:A total of67patients with suspected portal hypertension were enrolled in this study.Allthe patients were examined by multidetector CT and painless gastroscope，and the data werecomparatively analyzed.Result:Through the clinical symptoms, laboratory, abdomen color doppler ultrasound, painlessgastroscope inspection, MDCT check67patients are in line with liver cirrhosis portalhypertension of the diagnosis, including posthepatitic cirrhosis in45cases (hepatitis b andhepatitis c34patients in11cases), alcoholic cirrhosis11cases, biliary cirrhosis5cases(primary4cases, secondary1cases), and source cirrhosis in5cases, drug cirrhosis in1.MDCT and painless gastroscope two inspections separately detection esophageal varices46patients and50cases, diagnosis was68.7%and74.7%, both consistent rate was0.707.MDCT and painless gastroscope two inspection method detection stomach varicose veins(including mergers esophageal varices)58cases and36patients with diagnosis rate is86.6%and53.7%, both consistent rate was0.253.7patients do MDCT late-development existing.Conclusion:1. MDCT and painless gastroscope inspection on the total esophageal varices diagnosisconsistency is higher (K=0.707), is no statistical difference;2. MDCT and painless gastroscope inspection with stomach varicose veins diagnosisrate is consistent low, at0.253, both diagnosis was86.6%and53.7%, respectively, both havesignificant difference, MDCT better than painless gastroscope inspection.3. MDCT can observe the mucous membrane of the muscular layer, layer, serousmembrane or the outer layer of the arteries and veins, and clearly showed portal system andother collateral circulation and distribution of ZouXing, to prevent endoscopic treatment bythe organization glue ectopic embolism provide the scientific basis. This paper have sevenpatients underwent MDCT late-development existing tap, not endoscopic treatment, accounted for10.45%4. MDCT with stomach varicose veins diagnosis false positive, this paper has3casesaccounted for4.48%.5. Painless gastroscope inspection can look under the observation of varicosis vein ofmucous membrane surface parts, scope, degree, still can observe whether there are portalhypertension sex stomach trouble, stomach or duodenal ulcer and gastric cancer of the ball,polyps, bleeding, erosive gastritis, etc, be helpful for endoscopic treatment.
