| Objective: To study the relationship between endoscopic gastroscope performance and TCM syndromes in cirrhosis portal hypertensive gastropathy(PHG)by retrospective analysis method.Methods: This study was used the retrospective analysis method to consulted documents and related diagnostic criteria 128 patients with cirrhosis portal hypertensive gastropathy diagnosed by our hospital's endoscopy and eligible for inclusion criteria was collected from August 2016 to August 2017.We recorded their relevant clinical data,and then accorded to TCM syndrome differentiation and classification and observed the changes of gastric performance in patients by endoscopy.Then we explored the relationship between gastroscope performance of portal hypertension gastopathy and TCM syndromes,the course of cirrhosis,gastroesophageal varices,Helicobacter pylori(Hp)infection and tongue image.Results:1.In the 128 patients with cirrhosis portal hypertensive gastropathy,the incidence of male and female is 4.86:1,the average age of onset 50.81±10.14 years.2.The predisposing factors of cirrhotic cirrhosis portal hypertensive gastropathy are as follows: six exogenous pathogens and leisure disorders.3.There was no statistically significant difference between the mucosal manifestations and the syndromes of PHG under endoscope(P>0.05).The patients' gastric mucosa of damp-heat accumulation syndrome were mainly erosive,and secondly were congestion and edema.The patients' gastric mucosa of stagnation of liver Qi syndrome were mainly congestion and edema.The patients' gastric mucosa of damp-heat accumulation syndrome and obstruction of blood stasis syndrome were mainly mucosal bleeding.4.There was statistically significant difference between different syndromes of PHG and esophageal varices(P<0.05).The patients' degree of esophageal varices were mainly severe esophageal varices in syndrome of damp-heat accumulation,syndrome of blood stasis obstruction and syndrome of stagnation of liver Qi syndrome.5.The difference between the severity of PHG and the TCM syndromes is statistically significant(P<0.05),The patients' degree of PHG were mainly severe PHG in syndrome of obstruction of blood stasis,syndrome of stagnation of liver qi and damp-heat accumulation.6.The severity of PHG is positively correlated with the course of liver cirrhosis(P<0.05).With the increase of the course of cirrhosis,the severity of portal hypertensive gastropathy is aggravated.7.The difference between TCM syndromes of PHG and Hp infection is statistically significant(P<0.05),the rate of Hp infections in different TCM syndromes were as follows,syndrome of damp-heat accumulation(24.22%)> syndrome of stagnation of liver Qi(17.97%)>syndrome of obstruction of blood stasis(13.28%)>syndrome of liver and kidney Yin Deficiency(5.47%)>deficiency of spleen and kidney yang(3.13%).8.There was a correlation between gastric mucosa of PHG and tongue coating(P<0.05);The mucosal manifestations were mainly hyperemia and edema,and the tongue coating showed yellow-greasy moss(17.19%)>thin Huang(13.28%)>thin Bai(9.38%)>white-greasy moss(7.03%)>little moss or peeling moss(3.91%);The mucosal manifestations of erosion,flushing bleeding points,tongue coating performance is yellow greasy moss(28.91%)> thin Huang(10.15%)>white greasy moss(4.69%)>less moss or moss stripping Moss(3.13%)>Thin Bai(2.34%).Conclusions: The severity of portal hypertensive gastropathy and the course of cirrhosis and TCM syndromes,TCM syndromes and esophageal varices and Helicobacter pylori infection,as well as the endoscopic mucosal manifestations of portal hypertensive gastropathy and tongue image,there is a certain intrinsic relationship and regularity.Therefore,cirrhosis caused by portal hypertensive gastropathy,mild onset should be heat-relieving dampness,soothing liver qi and stomach;The severe disease should dispel evil and take care of the Fuzheng,promote blood circulation,dispel the liver and stomach in clinical.Patients with Hp infectious gastritis should be routinely treated with Hp. |
