Periodontitis is a chronic infectious disease of the periodontal tissues caused bydental plaque biofilm, its harms is no longer confined to the mouth. During the pastdecades, a large number of studies have shown that periodontal infection may be riskfactors for certain diseases, such as cardiovascular and cerebrovascular diseases(arteriosclerosis, myocardial infarction, stroke, etc.), diabetes, pregnancycomplications, respiratory infections, rheumatoid arthritis. Cerebral infarction, alsoknown as ischemic stroke, is the main types of stroke. Because of its high morbidityand high mortality rates, it has become one of the most important diseases thatendanger human health. Attemptting to provide a theoretical basis for periodontitisrole in the process of stroke, scholars have carried out a large number ofepidemiological investigations to explore the relationship between periodontal diseaseand stroke since1980s. However, due to differences in experimental design and thetechnical means, there are still different opinions in the academic community aboutthe exact relationship between them. We know that stroke can be divided intohemorrhagic stroke and ischemic stroke (cerebral infarction). The pathogenesis ofhemorrhagic stroke is significantly different from the cerebral infarction’s,inflammatory role in hemorrhagic stroke is far weaker than that in cerebral infarction.Therefore, there may be a closer relationship between periodontal infection andcerebral infarction.Objective:1. Explore the degree of periodontal infection in patients with acute cerebralinfarction, by detecting the serum Pg-IgG antibody of acute cerebral infarctionpatients and healthy subjects.2. Determine the effect of periodontal infection on the serum risk factors of acutecerebral infarction, by comparing Pg-IgG antibody-positive patients’ blood lipid,Hs-CRP and IL-6concentration with Pg-IgG antibody-negative patients’. Methods:1. Obtain the basic characteristics and medical history information of the subjects, byphysical examination and filling in the questionnaire.2. Extract subjects’ fasting blood in the next morning in hospital, part of the bloodwas sent to detect serum lipids and Fg; part of the blood was stored at-80°Ctemperature refrigerator after4°C centrifugation.3. Serum IgG antibody titers against Porphyromonas gingivalis were determinedwith the micro-ELISA method, sonicated freeze-dried bacteria diluted fluid ofPorphyromonas gingivalis ATCC33277was acted as antigen. The Porphyromonasgingivalis IgG titer was defined as the inverse value of the largest serial dilution.4. Detect the Hs-CRP, IL-6concentration of test serum by quantitative sandwichELISA with the use of Hs-CRP, IL-6ELISA kit.5. All data obtained from the experiment were analyzed by the Statistical PackageSPSS13.0.Results:1. The Pg-IgG antibody titers and antibody positive rate in the acute cerebralinfarction group were significantly higher than those in the control group (p <0.05); The Pg-IgG antibody titers in the acute cerebral infarction smokers andnon-smokers were also higher than those in the healthy control group smokers andnon-smokers(p <0.05). Compared with non-smokers, Pg-IgG antibody titers ofsmokers had no obvious change(p>0.05).2. The serum TC、TG、LDL、Lp (a)、Hs-CRP、Fg、IL-6levels significantly increasedin patients with acute cerebral infarction, and HDL level decreased (p <0.05or p<0.01); TC、LDL、Hs-CRP and IL-6levels in Pg-IgG antibodies positive patientswith acute cerebral infarction were significantly higher than those in Pg-IgGantibody negative patients with acute cerebral infarction(p <0.05).3. In analyses limited to acute cerebral infarction subjects, there were significantpositive correlations of Pg-IgG titer with TC、LDL、Hs-CRP and IL-6.Conclusion:1. Pg-IgG antibody levels in patients with acute cerebral infarction were higher thanthose in healthy controls, and the Pg-IgG antibody levels were not significantlyaffected by smoking. 2. The serum TC、TG、LDL、Lp(a)、Hs-CRP、Fg and IL-6average levels in patientswith acute cerebral infarction were significantly higher than those in healthysubjects, and HDL average level was lower than that in healthy subjects.3. The serum TC、LDL、Hs-CRP and IL-6average levels in Pg-IgG antibodiespositive patients with acute cerebral infarction were significantly higher than thosein Pg-IgG antibody negative patients with acute cerebral infarction. The Pg-IgGantibody levels in patients with acute cerebral infarction were positively correlatedwith serum TC、LDL、Hs-CRP and IL-6.
