Methods:select operation in patients with coronary heart disease in80cases, double blindrandomized into four groups. The four groups is the control group(group A) andexperimental group (group B1, group B2, group B3), each group is20cases. Thepatients of each group were given drugs random after they entering ICU, but nevergive them other drugs. Give the patients in group A a loading dose midazolam0.05mg/kg, after one minutes, give them continuous injection of midazolam in0.02-0.1mg/kg. Give the patients in experimental group with the same dosedexmedetomidine1ug/kg, after giving the patients in four group20min, giving thepatients in group B10.2ug/kg/h with micro pump, giving the patients in group B20.4ug/kg/h with micro pump, giving the patients in group B30.6ug/kg/h with micropump. All the drugs were stopped when the patients breath spontaneously. Recordthe data of b lood pressure (SBP, DBP), heart rate (HR), bispectral index (BIS)of the patients in four groups in patients entering ICU instantly (T1),30minuts aftergiving the drugs (T2), one hours after giving the drugs (T3), the patients recoverybreathing spontaneously (T4), ten minuts before extubation (T5), suction extubationafter extubation (T6), ten minuts after extubation (T7). And record the adversereaction, breathing time, extubation time and waking time.Results:1. There is no significant difference in patients of four groups in age, gender, weightand operation time (P>0.05).2. The changes of SBP, DBP, HR and BIS is great in the control group (group A)before and after extuation while the changes is fluctuation in the experimental group(group B1, B2, B3).3. The adverse reactions is great in extuation period in control group (group A), whilethe adverse reactions is less in awake extubation in experimental group (group B1, group B2, group B3).Conclution:1. The hemodynimics is stable during extubation while the dexmedetomidine weregiven the patients of cardiac OPCABG operation.2. The dexmedetomidine is dose-dependent sedative, analgesic effect, improve thepatients’ recovery.3. The dexmedetomidine of maintenance dose0.4ug/kg/h for postoperative sedationmoderate, stable hemodynamics, fewere adverse reactions,no affected for thepatients’ recovery time, more safe to apply in OPCABG patients with postoperativeextubation.
