Objective: To explore the influence and significance of hypoalbuminemiato clinical parameters and prognosis in elderly critical patients.Material: We enrolled160elder critical patients admitted in intensivecare unit (ICU) of the first hospital of JiLin University between January,2008and January,2011. Exclusion criteria was as follow:⑴serious digestiondysfunction.⑵ICU stays was shorter than72h.⑶Patient received albuminsupplementary treatment in latest24h.⑷The scores of acute physiology andchronic health evaluation were smaller than16.Methods: We detected concentration of albumin of all patients when theywere admitted.⑴We divided patients into death group and survival groupaccording to patients prognosis, then compared difference of plasma albumin inthese two groups.⑵We made statistical comparation of score of APACHEII,incidence rate of MODS, hospital stays and infection rate among normalalbumin group, mild hypoalbuminemia group, moderate hypoalbuminemiagroup and severe hypoalbuminemia group.⑶Statistical analysis above clinicalparameters after different nutritional support treatment. For normal distributionof measurement data, results were recorded the mean standard±deviation(X±S). Independent samples t-test was used in within group comparation,analysis of variance was used in comparation between groups. Count data wasrecorded by the number of cases and rate (n,%), X2test was used incomparation between groups, p<0.05as the test standards.Results:Concentration of albumin in survival group was higher than thatin death group(p<0.05);2) There were significantly difference of fungalinfection, MODS incidence, hospital stays, respirator-associated pneumonia and mortality between normal albumin group and hypoalbuminemia group.3)Combination enteral nutrition and parenteral nutrition therapy could effectivelyimprove the level of serum albumin and effective decrease incidence of adversereactions.Conclusion:The severity of the disease and mortality are closely related withconcentration of albumin in elderly critically patients.It is necessary to improve the patient’s serum albumin level bycombination various means of therapy as soon as possible.
